Term Description

Cold agglutinins are usually determined at refrigerator temperatures (4 degrees C). In positive cases, surgeons may request testing at additional temperatures, including 18 degrees C, room temperature (22 degrees C), and 28 degrees C, to determine how cold the patient's body temperature can be dropped during surgery. In cardiovascular surgery, the heart is cooled to 25°C to slow down metabolic processes. Since the blood of patients with a history of cold antibodies or cold hemagglutinin disease (such as Raynaud's) is likely to contain cold antibodies, such as anti-M, anti-I and anti-I, testing at various temperatures is performed in order to prevent an acute hemolytic episode.

A cold agglutinin is an autoimmune disease in which the body produces antibodies against red blood cells, causing the cells to agglutinate (clump together). These antibodies are usually IgM. The primary form of cold agglutinin disease is idiopathic. In other cases, the disease is a result of an underlying condition. Cold aggulitinin disease is known to occur in adults with a lymphoproliferative disease, such as lymphoma and chronic lymphoid leukemia, and in children with infections such as Mycoplasma pneumonia, mononucleosis, and HIV. Cold agglutinins affect many blood tests, such as red blood cell count, and by default, the mean corpuscular hemoglobin and mean corpuscular volume.
