Term Description

Neonate mental status is described as either completely limited (=4), very limited (=3), slightly limited (=2) or no impairment (=1) in the Neonatal Skin Risk Assessment Scale (NSRAS). Completely limited mental status indicates the neonate is unresponsive (does not flinch, grasp, moan, increase blood pressure or heart rate) to painful stimuli due to diminished level of consciousness or sedation. Very limited mental status is when the infant responds only to painful stimuli (flinches, grasps, moans, increased blood pressure or heart rate). Slightly limited mental status is when the infant is lethargic. No impairment is when the infant is alert and active. Only one numeric value (1, 2, 3 or 4) is selected.
