Neonatal Skin Risk Assessment Scale (NSRAS) Panel
Active
73789-0 General Physical Condition
Term Description
General physical condition of a neonate is based on their gestational age in the Neonatal Skin Risk Assessment Scale (NSRAS). Gestational age less than or equal to 28 weeks (=4) is described as very poor. Other gestation ages include greater than 28 weeks but less than or equal to 33 (=3), greater than 33 weeks but less than or equal to 38 weeks (=2), and greater than 38 weeks to posterm (=1). Only one numeric value (1, 2, 3 or 4) is selected.

73788-2 Mental Status
Term Description
Neonate mental status is described as either completely limited (=4), very limited (=3), slightly limited (=2) or no impairment (=1) in the Neonatal Skin Risk Assessment Scale (NSRAS). Completely limited mental status indicates the neonate is unresponsive (does not flinch, grasp, moan, increase blood pressure or heart rate) to painful stimuli due to diminished level of consciousness or sedation. Very limited mental status is when the infant responds only to painful stimuli (flinches, grasps, moans, increased blood pressure or heart rate). Slightly limited mental status is when the infant is lethargic. No impairment is when the infant is alert and active. Only one numeric value (1, 2, 3 or 4) is selected.

73787-4 Mobility
Term Description
Neonate mobility is described as either completely immobile (=4), very limited (=3), slightly limited (=2) or no limitations (=1) in the Neonatal Skin Risk Assessment Scale (NSRAS). Completely immobile is when the infant does not make even slight changes in body or extremity position without assistance (e.g. Pavulon). Very limited mobility is when the infant makes occasional slight changes in body or extremity but they are unable to make frequent changes independently. Slightly limited mobility is when the infant makes frequent though slight changes in body or extremity position independently. No limitations is when the infant makes major and frequent changes in position without assistance (e.g. turn head). Only one numeric value (1, 2, 3 or 4) is selected.

73786-6 Activity
Term Description
Neonate activity is described as either completely bed-bound (=4), Limited bed-bound (=3), slightly limited (=2) or unlimited (=1) in the Neonatal Skin Risk Assessment Scale (NSRAS). Completely bed-bound means the infant is in a radiant warmer with a clear plastic "saran" tent. Limited bed-bound means the infant is in a radiant warmer without a clear plastic "saran" tent. Slightly limited activity is when the infant is in a double wall isolette. Unlimited activity is when the infant is in an open crib. Only one numeric value (1, 2, 3 or 4) is selected.

73785-8 Nutrition
Term Description
Neonatal nutrition is described as either very poor (=4), inadequate (=3), adequate (=2) or excellent (=1) in the Neonatal Skin Risk Assessment Scale (NSRAS). Very poor nutrition means infant is NPO (Nil per os) and on intravenous fluids. Inadequate nutrition indicates the neonate receives less than optimum amount of liquid diet for growth (formula/breast milk) and is supplemented with intravenous fluids. Adequate nutrition is when the infant is on tube feedings which meet nutritional needs for growth. Excellent nutrition indicates the neonate bottle/breastfeeds every meal which meets nutritional needs for growth. Only one numeric value is selected.

73784-1 Moisture
Term Description
Moisture of neonatal skin is described as either constantly moist (=4), moist (=3), occasionally moist (=2) or rarely moist (=1) in the Neonatal Skin Risk Assessment Scale (NSRAS). Constantly moist skin is moist/damp every time the infant is moved or turned. Moist skin is often damp but not always; linens must be changed at least once a shift. Occasionally moist skin requires an extra linen change approximately once a day. Rarely moist skin is usually dry; linens require changing only every 24 hours. Only one numeric value is selected.

73783-3 Pressure Ulcer Risk Total
Term Description
Neonatal pressure ulcer risk total score is the cumulative value for each scored clinical condition parameter described in the Neonatal Skin Risk Assessment Scale (NSRAS). Neonates are at risk for skin breakdown (e.g. ulcers) if the total score is 16 or less.

73782-5 Pressure Ulcer Risk Evaluation
Term Description
Interpretation of the total score for the neonatal pressure ulcer risk evaluation [LOINC: 73783-3]. A critical cut-off score of 5 on the NSRASĀ© was determined to indicate neonates are "at risk" for skin breakdown. At a score of 5, the scale was found to be 83% sensitive and 81% specific, with 50% predictive value of positive scores and 95% predictive value of negative scores.
