Term Description

This term is used for assays that can detect, but not differentiate between the CTX-M-8 and CTX-M-25 extended spectrum beta lactamase genes in gram negative bacterial isolates. If either or both genes are positive, a single result of "positive" is reported. This term was created for, but is not limited in use to, the submitter's modified protocol for the Check-Points Check-MDR CT103XL assay.

Extended-spectrum beta-lactamases (ESBL) are enzymes produced by some bacteria that are resistant to the majority of beta-lactam antibiotics which include the penicillins, cephalosporins and the monobactam aztreonam. [https://www.uptodate.com/contents/extended-spectrum-beta-lactamases] Organisms producing CTX-M type ESBL enzymes have been isolated worldwide with more than 150 enzymes identified. These enzymes have been classified into seven major phylogenetic groups: CTX-M -1, CTX-M-2, CTX-M-8, CTX-M-9, CTX-M-25, CTX-M-74 and CTX-M-75. The rare ESBL type CTX-M-8 has been isolated in Germany, mainly in E. coli, but also in Salmonella and E. aerogenes. CTX-M type ESBL have been isolated worldwide and their incidence has increased dramatically and is still increasing leading to a major therapeutic issue. Currently more than 150 allelic variants of CTX-M have been identified. These enzymes are classified in five major phylogenetic groups based on their gene sequences: CTX-M -1, CTX-M-2, CTX-M-8, CTX-M-9, CTX-M-25; two additional groups have been reported recently: CTX-M-74 and CTX-M-75. The dissemination of these enzymes has caused an increased use of carbapenems for treatment of infections which carries its own risk of furthering resistance.
