Because cultures are sterile, diagnosis relies on other methodologies, including molecular techniques. [PMID:17889492] Such infections may involve native or prosthetic valves.

Tropheryma whipplei, formerly called Tropheryma whippeli, is a gram-positive bacillus that causes Whipple's disease. Both the organism and disease are named for Dr. George Whipple. Most people infected by T. whipplei are asymptomatic or have mild symptoms followed by a protective immune response. However, a small number develop Whipple's disease, which involves multiple organ systems, including the gastrointestinal system, heart and vascular system, central nervous system, and joints. Symptoms include abdominal pain and diarrhea, weight loss, arthralgias, and fever. Cardiac involvement includes endocarditis and myocarditis, and neurological manifestations span a wide range, including ataxia, seizures, supranuclear ophthalmoplegia, and delirium. [PMID:28722966] Molecular assays are used to diagnose T. whipplei in a variety of specimen types, including stool, blood, and tissue. Targets include the 16S rRNA gene and T. whipplei-specific repetitive DNA sequences. [PMID:28298472]
