Neutrophil cytoplasmic antibodies are autoantibodies that bind to the granulocytes of neutrophils. These antibodies were first identified in patients with Wegener's granulomatosis (WG) who also had systemic vasculitis and necrotizing crescentic glomerulonephritis. When viewed under a fluorescent microscope, a typical cytoplasmic fluorescent pattern is visible. These ANCA are called cytoplasmic ANCA (c-ANCA) and the antigen they target is proteinase 3 (PR3) which is part of the azurophilic granules of neutrophils. The perinuclear ANCA (p-ANCA) targets the antigen myeloperoxidase (MPO) and is associated with idiopathic forms of systemic vasculitis and glomerulonephritis. A third ANCA antibody is the atypical ANCA (a-ANCA). The presence of a-ANCA has been found in patients with inflammatory bowel diseases including ulcerative colitis (UC) and Crohn's disease (CD), autoimmune-mediated liver diseases, rheumatoid arthritis RA, and systemic lupus erythematosus (SLE).[PMID:27366026]
