Thyroglobulin (Tg) is a protein produced by and used entirely within the thyroid gland. Thyroglobulin is used by the thyroid gland to produce the thyroid hormones thyroxine (T4) and triiodothyronine (T3). Thyroglobulin is stored in the thyroid in follicle cysts surrounded by colloid cells. Via a reaction with the enzyme thyroperoxidase, iodine is covalently bound to tyrosine residues in thyroglobulin molecules, forming monoiodotyrosine (MIT) and diiodotyrosine (DIT). Thyroxine is produced by linking two moieties of DIT. Combining one particle of MIT and one particle of DIT produces triiodothyronine. Proteases in lysosomes digest iodinated thyroglobulin, releasing T3 and T4. Thyroglobulin levels in the blood can be used as a tumor marker for certain kinds of thyroid cancer. Thyroglobulin levels in the blood can also be elevated in cases of Graves Disease.

Thyroglobulin (Tg) is a glycoprotein homodimer produced by the thyroid gland. It is important in the synthesis of thyroxine and triiodothyronine, and is where the inactive forms of thyroid hormone and iodine are stored. Mutations in the Tg gene cause thyroid dyshormonogenesis resulting in goiter, and can be associated with moderate to severe congenital hypothyroidism. Polymorphisms in the Tg gene are associated with susceptibility to autoimmune thyroid diseases such as Graves disease or Hashimoto thyroiditis. Circulating immunoglobulins directed against thyroglobulin are commonly present in patients with autoimmune (Hashimoto) thyroiditis and, to a lesser extent, Graves disease. Thyroglobulin is a 660-kd protein precursor of thyroid hormones. Thyroglobulin antibodies are present in the sera of approximately 70% of patients with autoimmune thyroiditis. The principal use of the test is to confirm that a patient's diffuse goiter and/or hypothyroidism is due to autoimmune thyroiditis. The test may also help distinguish Graves disease from toxic nodular goiter. Although thyroglobulin antibodies are present less often than thyroid microsomal antibodies (eg, thyroid peroxidase antibodies, anti-TPO) in patients with autoimmune thyroid diseases, screening fro both immunoglobulins increases diagnostic sensitivity. Thyroglobulin antibodies may be detected in individuals without clinically significant thyroid disease. They do not define the patient's thyroid functional status.
