Toxoplasma gondii is an intestinal coccidium that multiplies within cats as definitive hosts as well as within many other mammals and birds as intermediate hosts. T. gondii reproduces sexually only in cats. It infects the intestinal epithelium and produces oocysts which are shed in the feces. Humans acquire infections most often by consumption of undercooked meat containing tissue cysts or food contaminated by cat feces. Less than 0.1 percent of human cases are acquired congenitally. Toxoplasmosis is minor and self-limiting in immunocompetent individuals who may be asymptomatic or present with fever, lymphadenopathy, muscle aches and headache. Toxoplasma may manifest as encephalitis in immunosuppressed individuals including patients with acquired immune deficiency syndrome (AIDS) and may lead to coma or death. T. gondii often infects the brain and retina in congenitally infected children. Mild symptoms include slightly diminished vision while severely diseased children exhibit a pattern of retinochoroiditis, hydrocephalus, convulsions and intracerebral calcifications. Ocular disease may also occur. Toxoplasma is diagnosed through serology and upon histologic examination of tissues. Prevention includes thorough cooking of meat and proper management of cats. Herpes simplex virus 1 and 2 (HSV-1 and HSV-2) are two species of the herpes virus family, Herpesviridae, which cause infections in humans. Eight members of herpes virus family infect humans and cause a variety of illnesses including cold sores, chickenpox or varicella, shingles or herpes zoster (VZV), cytomegalovirus (CMV). Some herpes viruses have been implicated in certain cancers and in the inflammation of the brain (encephalitis). All viruses in the herpes family produce life-long infections.
They are also called Human Herpes Virus 1 and 2 (HHV-1 and HHV-2) and are neurotropic and neuroinvasive viruses; they enter and hide in the human nervous system, accounting for their durability in the human body. HSV-1 is commonly associated with herpes outbreaks of the face known as cold sores or fever blisters, whereas HSV-2 is more often associated with genital herpes.

It contains a linear double-stranded DNA encased in an icosahedral capsid that is surrounded by a viral envelope. It replicates slowly and may take weeks for the infection to become visible microscopically. Its life cycle includes a lytic and a latent phase. During the lytic phase the cytoskeleton of the infected cell is disrupted and becomes very large. The enlarged cell eludes detection by natural killer (NK) cells by encoding UL16. UL16 is a protein that binds to the ligand NDG2D which is a surface receptor on NK cells and, when blocked, prevents the CMV infected cell from apoptotic signaling to NK cells to destroy it.[https://pedclerk.bsd.uchicago.edu/page/cytomegalovirus-cmv] In the US one in three children are infected by CMV by the age of five and over half of adults are infected by age 40. Infection with CMV is mostly asymptomatic. Symptoms when present include fever, sore throat, fatigue, swollen glands. CMV can occasionally lead to mononucleosis or hepatitis. Immunocompromised patients experience much more acute symptoms that can affect the eyes, lungs, liver, esophagus, stomach, and intestines. For babies born with CMV, involvement with the brain, spleen, lung, liver can occur. In addition, there may be growth problems and hearing loss which is the most common symptom in congenital CMV infection. CMV is transmitted in urine, saliva, blood, tears, semen and breast milk by direct contact. Rubella (also known as epidemic roseola, German measles, liberty measles or three-day measles) is a disease caused by the Rubella virus. It is often a mild illness, with symptoms of sore throat, low-grade fever, and a rash on the body. The virus usually enters the body through the nose or throat. Like most viruses living along the respiratory tract, it is transmitted from person to person by tiny droplets that are inhaled. Rubella can pose a serious risk if transmitted from a mother to her developing baby if a woman is infected while she is pregnant, especially during the first trimester. The baby is at risk for having congenital rubella syndrome (CRS). Alpha-1 antitrypsin deficiency is a relatively common autosomal recessive condition. The most common deficiency alleles in the alpha-1-antitrypsin gene on the protease inhibitor locus are PI*Z and PI*S. The normal allele is PI*M. Patients homozygous for PI*Z or PI*S or heterozygous PI*Z/PI*S will have decreased alpha-1-antitrypsin levels, with PI*Z/PI*Z having lowest levels of 10-20% of normal. Patients with PI*Z/PI*Z or PI*S/PI*Z genotypes reportedly have increased risk of childhood liver disease and chronic obstructive pulmonary disease(COPD) and emphysema in adult life. PI*M/PI*Z patients may have decreased pulmonary function and increased risk for COPD, especially smokers.

Used to diagnose hereditary hypo α1-antitrypsin (α1AT). An increased prevalence of non-MM phenotypes is found with cryptogenic cirrhosis and with CAH. Cirrhosis in a child should raise consideration of α1AT deficiency or Wilson's disease. Diagnosis of inflammatory states, if elevated (eg, rheumatoid arthritis, bacterial infection, vasculitis, neoplasia). Alpha 1-antitrypsin or α1-antitrypsin (A1AT) is a serine protease inhibitor (serpin). It protects tissue from enzymes from inflammatory cells, especially elastase, and is present in human blood at 1.5 - 3.5 gram liter.
