Part Descriptions
LP173523-4 Bacterial carbapenem resistance blaNDM gene
New Delhi Metallo-beta-lactamase (NDM) is an enzyme that makes bacteria resistant to a broad range of beta-lactam antibiotics, including those in the carbapenem family. NDM-1 presents as the most dominant type. The NDM gene encodes beta-lactamase enzymes called carbapenemases. The most common bacteria that make the NDM enzyme are Gram-negative such as Escherichia coli and Klebsiella pneumoniae, but the gene for NDM can spread from one strain of bacteria to another by horizontal gene transfer.

- The PrThr property is used for LOINC terms whose results are reported using an ordered categorical scale, regardless of whether or not an internal threshold was used to make that determination. This change was approved by the Laboratory LOINC Committee in June 2016.; Corrected Component from "Bacterial carbapenemase resistance" to "Bacterial carbapenem resistance" (because the bacteria are resistant to carbapenem antibiotics by virtue of producing carbapenemase) and removed parentheses around and within the gene name according to recommended nomenclature [PMID: 25249841]; Updated System from "Isolate" and Method from "Probe.amp.tar" according to the new, more sustainable model for LOINC molecular resistance terms [Approved by LOINC Laboratory Committee in June 2018].

72421-1 Vancomycin resistance vanB gene [Presence] by Molecular method
Part Descriptions
LP157337-9 Bacterial vancomycin resistance vanB gene
This test is used for the detection of vancomycin resistance genes by multiplex PCR. Vancomycin is a glycopeptide antibiotic used to treat various infections, primarily those caused by gram-positive bacteria. Vancomycin resistance genes were first found in enterococci, and more recently have been found in Staphylococcus aureus. The emergence of vancomycin-resistant enterococci (VRE) and vancomycin-resistant Staphylococcus aureus (VRSA) in hospitalized patients has lead to a restricted use of vancomycin. To date, multiple vancomycin resistance genes have been characterized, including: vanA, vanB, vanC (vanC-1, vanC-2, vanC-3), and VanD. Of the six van types, vanA and vanB are the most resistant to vancomycin, and are the two types that cause VRSA. PMID: 24983424

49617-4 Carbapenem resistance blaKPC gene [Presence] by Molecular method
Part Descriptions
LP64707-0 Bacterial carbapenem resistance blaKPC gene
Non-susceptibility to carbapenems in gram-negative bacilli by means of the enzyme KPC (Klebsiella pneumoniae carbapenemase) is becoming more common. The gene beta-lactamase KPC (bla(KPC)) encodes KPC enzyme production, which in turn hydrolyzes and inactivates carbapenem antibiotics. KPC enzymes are a class A beta lactamases with several variants (KPC-2, KPC-3, etc) known. Various methods are used to detect specific portions of genes encoding KPC production. Carbapenemases can also be detected by other methods, including the modified Hodge test to detect carbapenemase production in isolates and minimum inhibitory concentration (MIC) to determine level of resistance.

35492-8 Methicillin resistant Staphylococcus aureus (MRSA) DNA [Presence] in Specimen by NAA with probe detection
Part Descriptions
LP34425-6 Staphylococcus aureus.methicillin resistant
Methicillin resistant Staphylococcus aureus (MRSA) is a staph infection that is resistant to multiple antibiotics. MRSA frequently causes skin infections, but can also cause pneumonia and, if untreated, can lead to sepsis. MRSA is spread when a person comes in contact with an infected wound or by sharing personal items with a person infected with MRSA. The risk of infection is increased in crowded spaces such as gyms, daycare centers, military bases, and patients being treated as inpatients in medical facilities. Studies have shown that one in three people are asymptomatic carriers of staph in their nose and that two people in 100 are carriers of MRSA. To prevent contracting MRSA, maintain good personal hygiene by frequently hand washing and bathing often, especially after exercise. Keep scrapes and open wounds covered and get medical attention for skin infections. MRSA infection of the skin may initially resemble a spider bite, but can become red, swollen, painful, warm to the touch, filled with pus, and accompanied by a fever.[https://www.cdc.gov/mrsa/index.html]

95809-0 Carbapenem resistance genes [Presence] by Molecular method
Term Description
Qualitative detection of one or more carbapenemase resistance genes. To report the resistance gene(s) found, the carbapenem resistance genes identified term [LOINC: 63368-5] can be used.
Source: Regenstrief LOINC
Part Descriptions
LP113695-3 Carbapenem resistance genes
Carbapenems are stable to AmpC β-lactamases and extended-spectrum-β-lactamases. Carbapenemases are a diverse group of b-lactamases that are active not only against the oxyimino-cephalosporins and cephamycins but also against the carbapenems. Aztreonam is stable to the metallo-β-lactamases but many IMP and VIM producers are resistant, owing to other mechanisms. Carbapenemases were formerly believed to derive only from classes A, B, and D, but a class C carbapenemase has been described.
IMP-type carbapenemases (one of the metallo-β-lactamases)(class B): Plasmid-mediated IMP-type carbapenemases, 17 varieties of which are currently known, found in enteric Gram-negative organisms, Pseudomonas and Acinetobacter species.
VIM (Verona integron-encoded metallo-β-lactamase): A second growing family of carbapenemases, the VIM family, VIM enzymes occur mostly in P. aeruginosa, also P. putidaand, very rarely, Enterobacteriaceae.
Both IMP and VIM hydrolyse all β-lactams except monobactams, and evade all β-lactam inhibitors.
OXA (oxacillinase) group of β-lactamases (Class D): The OXA group of β-lactamases occur mainly in Acinetobacter species and are divided into two clusters. OXA carbapenemases hydrolyse carbapenems very slowly in vitro, and the high MICs seen for some Acinetobacter hosts (>64 mg/L) may reflect secondary mechanisms. They are sometimes augmented in clinical isolates by additional resistance mechanisms, such as impermeability or efflux. OXA carbapenemases also tend to have a reduced hydrolytic efficiency towards penicillins and cephalosporins.
KPC (K. pneumoniae carbapenemase) (Class A): A few class A enzymes, most noted the plasmid-mediated KPC enzymes, are effective carbapenemases as well. Ten variants, KPC-2 through KPC-11 are known, and they are distinguished by one or two amino-acid substitutions
The class A Klebsiella pneumoniae carbapenemase (KPC) is currently the most common carbapenemase.
CMY (Class C): The first class C carbapenemase was described in 2006 and was isolated from a virulent strain of Enterobacter aerogenes. It is carried on a plasmid, pYMG-1, and is therefore transmissible to other bacterial strains.

92968-7 Methicillin resistance mecA+mecC genes [Presence] in Isolate or Specimen by Molecular genetics method
Part Descriptions
LP249930-1 Bacterial methicillin resistance mecA+mecC genes
The mecA gene and its homologue, mecC, are found in bacterial cells and enable bacteria to be resistant to antibiotics such as penicillin, oxacillin and other penicillin-like antibiotics. These genes are carried on a mobile genetic element called the staphylococcal cassette chromosome mec (SCCmec), which integrates at an open reading frame (orfX) in the bacterial genome. The most commonly known carrier of the mecA gene is methicillin-resistant Staphylococcus aureus (MRSA). The prevalence of mecC MRSA is currently low but varies geographically where in some regions its presence is increasing. PMID: 24331435

86714-3 Carbapenem resistance blaOXA-51-like gene [Presence] by Molecular method
Part Descriptions
LP249792-5 Bacterial carbapenem resistance blaOXA-51-like gene
The carbapenemase blaOXA-51-like genes are members of the beta lactam resistance gene group OXA, which causes resistance to carbapenem antibiotics. The blaOXA-51-like genes are primarily found in Acinetobacter baumannii, which is a common cause of multidrug-resistant nosocomial infections. The blaOXA-51- and blaOXA-23-like genes are the two most common resistance genes in A. baumannii and cause resistance to drugs such as ceftazidime, piperacillin, cefepime, nitrofurantoin and ertapenem. PMID: 26550338 The "OXA-like" groups of genes are molecularly similar to and thought to evolve from the original gene for which the group is named. For example, as of 2014 the blaOXA-23-like group included 19 genes, including including blaOXA-23, -27, -49, and -73. PMID: 24696435

86221-9 Colistin resistance mcr-1 gene [Presence] by Molecular method
Part Descriptions
LP249101-9 Bacterial colistin resistance mcr-1 gene
The mobilized colistin resistance genes (mcr-1 to mcr-5, and variants) are located on a plasmid within bacterial cells and affect the ability of antibiotics, including polymyxins and colistin, to bind to the bacteria. The mode of action of the antibiotics is to bind to the outer membrane of the phosphate group of lipid A lipopolysaccharide and to intercalate into the inner membrane forming pores that result in bacterial lysis. The polymyxins & colistin are the last line of defense used to treat multidrug resistant gram-negative bacteria so the emergence of the mcr genes in Enterobacteriaceae, including Klebsiella pneumoniae, is onerous and challenging to public health. 86621-0 Methicillin resistance mecC gene [Presence] by Molecular method
Term Description
Detection of the mecC gene in methicillin-resistant bacteria, specifically methicillin-resistant staphyloccus aureus (MRSA), in various clinical specimens.
Source: Regenstrief LOINC
Part Descriptions
LP249928-5 Bacterial methicillin resistance mecC gene
The mecC gene, a homologue of mecA, is found in bacterial cells and enable bacteria to be resistant to antibiotics such as oxacillin. The gene is carried on a mobile genetic element called the staphylococcal cassette chromosome mec (SCCmec), which integrates at an open reading frame (orfX) in the bacterial genome. Detection of the gene in clinical specimens suggests the presence of methicillin-resistant bacteria, primarily Staphylococcus aureaus (MRSA). The prevalence of mecC MRSA is currently low but varies geographically where in some regions its presence is increasing. PMID: 24331435

LP265773-4 Bacterial ABXBACT probe.amp.tar
PCR-based Methods for bacterial susceptibility are similar to genotyping for viral susceptibility, in that resistance or susceptibility is inferred based on the presence or absence of specific genetic information that is known to encode resistance to a specific antibiotic or group of antibiotics. For bacteria, resistance is often conferred by genes that are carried on plasmids, which can be transferred from one species to another. These genes encode proteins that specifically inhibit certain antibiotics, such as carbapenems or aminoglycosides.

85828-2 Carbapenem resistance blaOXA-58-like gene [Presence] by Molecular method
Term Description
This test reports the presence of the OXA-58-like bacterial carbapenemase resistance type gene in gram negative bacterial isolates. This term was created for, but is not limited in use to, the submitter's modified protocol for the Check-Points Check-MDR CT103XL assay.
Source: Regenstrief LOINC
Part Descriptions
LP248276-0 Bacterial carbapenem resistance blaOXA-58-like gene
The Ambler classification for carbapenemases utilizes four molecular classes - A, B, C and D. The OXA group of β-lactamases occur mainly in Acinetobacter species and are part of molecular class D. The "OXA-like" groups of genes are molecularly similar to and thought to evolve from the original gene for which the group is named. For example, as of 2014 the blaOXA-23-like group included 19 genes, including including blaOXA-23, -27, -49, and -73. PMID: 24696435. Many OXA enzymes (OXA-23-like, OXA-24-like, OXA-48-like, OXA-58-like) are considered to be responsible for the worldwide resistance epidemics. OXA-23-like enzymes have been isolated in carbapenem-resistant Acinetobacter baumannii (CR-AB) in both the US and in China, revealing the importance of national and global efforts to control the spread of CR-AB.

Part Descriptions
LP248274-5 Bacterial carbapenem resistance blaOXA-48-like gene
The existence of OXA-48 like class D beta-lactamases has been increasing in enterobacter species organisms. The "OXA-like" groups of genes are molecularly similar to and thought to evolve from the original gene for which the group is named. For example, as of 2014 the blaOXA-23-like group included 19 genes, including including blaOXA-23, -27, -49, and -73. PMID: 24696435. The OXA-48 like class D beta-lactamase enzymes are highly resistant to penicillins and beta lactamase inhibitors and show a low resistance to carbapenems. These varied OXA-48-like beta-lactamases have been identified worldwide, including OXA-48-like producing Klebsiella pneumoniae with resistance to multiple drugs that emerged in Jordanian hospitals. Reports have shown that organisms producing OXA-48 like enzymes are either not resistant to broad-spectrum cephalosporins or show a decreased susceptibility to carbapenemase drugs. Identifying them can be a challenge. Screening and detection is required to prevent and control dissemination. PMID: 22499996 PMID: 26993738

LP248287-7 Bacterial carbapenem resistance blaGES gene
Extended-spectrum beta-lactamases (ESBL) are enzymes produced by some gram negative bacilli that confer resistance to the majority of beta-lactam antibiotics, include the penicillins, cephalosporins and the monobactam aztreonam. ESBLs are most often plasmid-borne. Specific ESBLs include TEM, SHV, and the CTX-M groups, which are the most clinically prevalent, and VEB, PER, BEL, and GES, which are less common. PMID: 19246801 -TEM and SHV subtypes are derived from amino acid substitutions, resulting in enzymatic hydrolysis of many beta-lactam antibiotics. -CTX-M genes originate from Kluyvera species. They are separated based on their amino acid sequences into the following groups: CTX-M-1, CTX-M-2, CTX-M-9, CTX-M-8, and CTX-M-25. -VEB, PER, BEL, and GES genes are less common. [Mayo Medical Laboratories] The nomenclature for ESBL genes includes the prefix "bla", which stands for "beta-lactam", followed by the enzyme (ie. blaTEM).

Part Descriptions
LP248277-8 Bacterial carbapenem resistance blaVIM gene
Verona integron-encoded metallo-β-lactamase (VIM) is part of a growing family of carbapenemases and includes at least 10 members. VIMs are one of the most common types of carbapenemases, with many variants in at least 23 species of Gram-negative bacilli from more than 40 regions of the world. PMID: 21449842 Studies have shown that a high percentage of patients with immunodeficiency and drug resistant nosocomial infections are infected with a VIM or IMP producing organism, particularly when the organism identified is Pseudomonas aeruginosa. PMID: 27563634 The two genes are often present together although they can also be present individually. Identifying the presence of these genes is important in monitoring drug resistance and determining the best antibiotic therapy methods. PMID: 28085553

Plasmid-mediated IMP-type carbapenemases are currently known to have 17 varieties. Studies have revealed its establishment in Japan in the 1990s in enteric Gram-negative organisms, Pseudomonas species and Acinetobacter species. IMP genes usually co-exist with other resistance genes resulting in multi-drug resistance.
Studies have shown that a high percentage of patients with immunodeficiency and drug resistant nosocomial infections are infected with a VIM or IMP producing organism, particularly when the organism identified is pseudomonas aeruginosa. The two genes are often present together although they can also be present individually. Identifying the presence of these genes is important in monitoring drug resistance and determining the best antibiotic therapy methods.
