Iron homeostasis diseases are common and may be attributed to iron deficiency or iron overload. Iron deficiency may be due to increased loss or inadequate absorption. Iron overload occurs when the erythropoiesis is normal, but the iron exceeds the iron binding capacity of transferrin. Iron overload may also occur when catabolism of red cells occurs. Iron levels are used to help diagnose specific types of anemia. Iron levels are low in iron deficiency anemia and anemia of chronic disease. Iron levels are normal to increased in sideroblastic anemia and increased in hemolytic anemias,hemochromatosis, and acute liver disease. Normal to decreased iron levels are seen with protein depletion. Other tests used as iron status indicators include ferritin, transferrin, TIBC, and iron saturation.(Jacobs and DeMott Laboratory Test Handbook, 5th edition, DS Jacobs, WR DeMott and DK Oxley, 2001.)
In veterinary toxicology, one of the best known types of iron poisoning is in young piglets infected with an iron dextran preparation for the prevention of postnatal anemia. However, iron intoxication can also occur in other animal species.

Erythrocytes or red blood cells (RBCs) are the cells in the circulation that carry oxygen to and remove carbon dioxide from the tissues throughout the body. They are produced in the bone marrow in response to erythropoietin where they transition through six stages over a seven day period. When they are released into the circulation, their nucleus has been extruded and they measure 6-8 microns in diameter. The lifespan of RBCs is about 120 days. When the RBC ages the cell membrane becomes less pliable and tears as they cell travels through the micro vessels of the body. The damaged RBCs are removed from the circulation by the spleen. Variations in number, shape and size of RBCs are indicative of many diseases and disorders. There are many factors that may lead to decreased numbers of RBCs including hemorrhage, hemolysis, iron or vitamin deficiency, marrow failure and more. Term Description
The hemoglobin content of RBCs (MCH) has long been one of the indices reported as part of the complete blood count. However, mature erythrocytes are not sensitive to recent iron homeostasis. The hemoglobin content of reticulocytes, RBC precursors, has more recently been proposed as a better indicator of iron deficiency and early response to iron therapy (Am J Hematol. 2008 Apr;83(4):307-10.)

Reticulocytes are immature red blood cells (RBC), comprising about 1% of the red cells in the human body. They develop and mature in the bone marrow and then circulate for about a day in the blood before developing into mature RBC. Like mature RBC, reticulocytes do not have a nucleus. They are called reticulocytes because of a reticular (mesh-like) network of ribosomal RNA that is visible under a microscope. Reticulocytes appear slightly bluer than other RBC when stained with Romanowsky stain. Reticulocytes are also slightly larger and thus can be identified as a high mean corpuscular volume during a full blood count done by machine.

In humans, reticulocytes are erythroid cells that have just undergone extrusion of their nucleus. They still contain some organelles that gradually decrease in number as the cells mature. Ribosomes are last to disappear. Certain staining techniques cause components of the ribosomes to precipitate into characteristic "reticulum" (not the same as the endoplasmic retuculum), hence the name reticulocytes.

Reticulocytes are immature red blood cells (RBC), comprising about 1% of the red cells in the human body. They develop and mature in the bone marrow and then circulate for about a day in the blood before developing into mature RBC. Like mature RBC, reticulocytes do not have a nucleus. They are called reticulocytes because of a reticular (mesh-like) network of ribosomal RNA that is visible under a microscope. Reticulocytes appear slightly bluer than other RBC when stained with Romanowsky stain. Reticulocytes are also slightly larger and thus can be identified as a high mean corpuscular volume during a full blood count done by machine.

In humans, reticulocytes are erythroid cells that have just undergone extrusion of their nucleus. They still contain some organelles that gradually decrease in number as the cells mature. Ribosomes are last to disappear. Certain staining techniques cause components of the ribosomes to precipitate into characteristic "reticulum" (not the same as the endoplasmic retuculum), hence the name reticulocytes.

Ferritin is the major iron storage protein in mammals, and is located primarily in the liver, spleen and bone marrow. Serum ferritin concentrations correlate well with total body iron stores in normal subjects and in patients with iron deficiency or transfusional iron overloads. The measurement of serum ferritin is now considered to be an accurate, quantitative, and convenient method of assessing total body iron stores. Low serum ferritin is diagnostic for iron deficiency anemia, and elevated serum ferritin occurs in the thalassemia but can also occur in other acute and chronic conditions such as hepatic disease, respiratory infections and certain types of malignancy.
