Xanthochromia refers to the yellowish appearance of cerebrospinal fluid in certain medical conditions, especially subarachnoid hemorrhage, when red blood cells (RBC) enter the CSF during bleeding. RBCs are eventually destroyed, releasing the oxygen-carrying molecule heme, which in turn is degraded to yield bilirubin, a yellow-green pigment. Heme that enters the CSF for different reasons (for example when a small blood vessel is damaged during a lumbar puncture), has no time to be digested and bilirubin is not generated. Many laboratories rely on the color of the fluid alone when testing for xanthochromia. Recent guidelines, however, suggest that spectrophotometry should be performed since bilirubin absorbs light of wavelengths between 450-460 nm. Two related substances, oxyhemoglobin and methemoglobin (410-418 nm and 403-410 nm absorption ranges, respectively) that are also released when heme is metabolized may be detected during this process. There are other causes for xanthochromia.

The LOINC "Body fld" System represents body fluids other than serum, plasma, blood, urine, and cerebrospinal fluid. When a LOINC term with the System "Body fld" is used, the specific specimen source or body fluid type should be reported elsewhere with the result. Where more specific terms for pleural, peritoneal, synovial fluid, etc. exist, we encourage you to use the more specific terms. In contrast to "Body fld", the System "Asp" in LOINC represents the fluid withdrawn during an aspiration procedure from an abnormal collection of fluid, such as from an abscess or cyst. "Asp" is used for abnormal fluid collections that are aspirated, while "Body fld" is used for named body fluids, such as synovial fluid or peritoneal fluid, that are not inherently pathologic.
