Carboxyhemoglobin, a complex of hemoglobin and carbon monoxide, is a marker of exposure to inhaled carbon monoxide (CO). CO gas is produced from almost any fire as the result of incomplete combustion to CO2, and the normal individual will have about 1 or 2 % of their hemoglobin in the form of carboxyhemoglobin, and up to 5% depending on environmental circumstances. Smokers can have levels as high as 10% or even higher. Poisoning with CO generally results from improperly vented furnaces, automobile exhaust, and fires, with carboxyhemoglobin:hemoglobin ratios of 10-30% found in symptomatic individuals and 30-90% in fatalities. With a 230 fold greater affinity, inhaled CO avidly replaces oxygen at its hemoglobin binding site. In addition, replacing even one of the 4 sites of the hemoglobin tetramer with CO prevents dissociation of the remaining 3 oxygen moieties to tissue (known as left shift of O2 dissociation curve). As a result, hemoglobin remains red and skin color is rosy despite physiologic anoxia. Although measuring carboxyhemoglobin can confirm carbon monoxide poisoning, the level does not always correlate with the clinical picture; carbon monoxide poisoning involves other mechanisms as well, including binding to myoglobin, cytochrome oxidase, and direct effects on the central nervous system.
