Term Description
This panel is the traditional hemogram plus platelet count which must now be reported with with hemograms according to current US re-imbursement rule The panel includes 2 different RDWs to accommodate the two different ways of reporting them. (Most automated instruments report as a percent ). The hemoglobin produced by the automatic counters does not use a counting method to generate the hemoglobin so we have used the methodless version of hemoglobin in this panel.

Leukocytes or white blood cells (WBCs) are immune cells that fight infection, neoplasms and other inflammatory conditions, and mediate allergic responses. There are five types of WBCs normally present in the circulation that are all derived from a similar stem cell in the bone marrow. The five type of WBCs are divided into two groups based on the presence or absence of granules in the cytoplasm. The granulocytes include the neutrophils, basophils and eosinophils. The non-granulocytes include the lymphocytes and the monocytes. The neutrophils fight infection by ingesting and digesting bacteria. Eosinophils and basophils respond to allergic reactions and are capable of ingesting antigen-antibody complexes. Monocytes phagocytose bacteria and release interferon to stimulate the immune system. Lymphocytes are divided into T-cells and B-cells. T-cell immunity is cellular and involves the activation of phagocytes and B-cell immunity uses antibodies to fight infection. Both elevated and low leukocyte counts can be markers of infection and malignancy, and low leukocyte counts are associated with a variety of primary and secondary immunodeficiencies, depending on the WBC type(s) that are out of range. Erythrocytes or red blood cells (RBCs) are the cells in the circulation that carry oxygen to and remove carbon dioxide from the tissues throughout the body. They are produced in the bone marrow in response to erythropoietin where they transition through six stages over a seven day period. When they are released into the circulation, their nucleus has been extruded and they measure 6-8 microns in diameter. The lifespan of RBCs is about 120 days. When the RBC ages the cell membrane becomes less pliable and tears as they cell travels through the micro vessels of the body. The damaged RBCs are removed from the circulation by the spleen. Variations in number, shape and size of RBCs are indicative of many diseases and disorders. There are many factors that may lead to decreased numbers of RBCs including hemorrhage, hemolysis, iron or vitamin deficiency, marrow failure and more. Platelets are miniature fragments of cytoplasm that break off of megakaryocytes. Their primary function is to inhibit excessive hemorrhage after blood vessel injury. Platelets cling to the damaged part of the blood vessel where they become activated, recruit other platelets to the site, and begin the cascade of physical and chemical events that lead to clot formation. Activated platelets secrete cytokines, chemokines, and growth factors, all which play a role in thrombus development.When activated, platelets stimulate monocytes and neutrophils to leave the blood vessel and enter the tissue. PMID: 27226790 The normal range of platelets is 150,000 to 450,000 platelets/uL. Lower than normal platelet counts can lead to increased bleeding and is caused by a number of factors including: chemotherapy, autoimmune disorders and some medications. Increased platelets can cause hypercoagulation due to cancer, anemia, spleen removal and some medications.[MedlinePlus Encyclopedia:003647]

A lymphocyte is a type of white blood cell in the vertebrate immune system.
A lymphocyte count is usually part of a peripheral complete blood cell count and is expressed as percentage of lymphocytes to total white blood cells counted. A general increase in the number of lymphocytes is known as lymphocytosis whereas a decrease is lymphocytopenia.
An increase in lymphocyte concentration is usually a sign of a viral infection (in some rare case, leukemias are found through an abnormally raised lymphocyte count in an otherwise normal person).
A low normal to low absolute lymphocyte concentration is associated with increased rates of infection after surgery or trauma.

A monocyte is a mononuclear leukocyte, intimately involved in the initiaton of an immune response. It also protects against blood-borne pathogens and migrates quickly to sites of infection in the tissues. Monocytes are usually identified in stained smears by their large bilobed nucleus. They are produced by the bone marrow from hematopoietic stem cell precursors called monoblasts. The monoblast is the first stage of monocyte maturation. The developmental stages (4 subtypes) of the monocytes are: monoblast, promonocyte, immature monocyte, and mature monocyte.

White blood cells formed in lymphoid tissue. The nucleus is round or ovoid with coarse, irregularly clumped chromatin whereas the cytoplasm is typically pale blue with azurophilic (if any) granules. Most lymphocytes can be classified as either T cells (transit through the thymus), B cells or natural killer cells (NK).

A lymphocyte is a type of white blood cell in the vertebrate immune system.
A lymphocyte count is usually part of a peripheral complete blood cell count and is expressed as percentage of lymphocytes to total white blood cells counted. A general increase in the number of lymphocytes is known as lymphocytosis whereas a decrease is lymphocytopenia.
An increase in lymphocyte concentration is usually a sign of a viral infection (in some rare case, leukemias are found through an abnormally raised lymphocyte count in an otherwise normal person).
A low normal to low absolute lymphocyte concentration is associated with increased rates of infection after surgery or trauma.

A monocyte is a mononuclear leukocyte, intimately involved in the initiaton of an immune response. It also protects against blood-borne pathogens and migrates quickly to sites of infection in the tissues. Monocytes are usually identified in stained smears by their large bilobed nucleus. They are produced by the bone marrow from hematopoietic stem cell precursors called monoblasts. The monoblast is the first stage of monocyte maturation. The developmental stages (4 subtypes) of the monocytes are: monoblast, promonocyte, immature monocyte, and mature monocyte.

LP14820-2 Hepatitis B virus surface
The hepatitis B surface antigen (HBsAg) is most frequently used to screen for the presence of this infection. It is the first detectable viral antigen to appear during infection. However, early in an infection, this antigen may not be present and it may be undetectable later in the infection as it is being cleared by the host. The infectious virion contains an inner "core particle" enclosing viral genome. The icosahedral core particle is made of 180 or 240 copies of core protein, alternatively known as hepatitis B core antigen, or HBcAg. During this 'window' in which the host remains infected but is successfully clearing the virus, IgM antibodies to the hepatitis B core antigen (anti-HBc IgM) may be the only serological evidence of disease. Therefore most hepatitis B diagnostic panels contain HBsAg and total anti-HBc(both IgM and IgG).

Some indicate that the confirmatory test requires and an extra charge, some do not. We believe that at present (2011) the confirmatory test is usually a neutralization test. But only one lab that we reviewed specified the method as such. This term covers all confirmatory methods and will not require changing if/when confirmatory methods change.

Rubella (also known as epidemic roseola, German measles, liberty measles or three-day measles) is a disease caused by the Rubella virus. It is often a mild illness, with symptoms of sore throat, low-grade fever, and a rash on the body. The virus usually enters the body through the nose or throat. Like most viruses living along the respiratory tract, it is transmitted from person to person by tiny droplets that are inhaled. Rubella can pose a serious risk if transmitted from a mother to her developing baby if a woman is infected while she is pregnant, especially during the first trimester. The baby is at risk for having congenital rubella syndrome (CRS). The most common symptoms of CRS include heart defects, deafness, cataracts, intellectual disabilities, and liver or spleen damage.[CDC: Rubella]

The ABO blood group antigens are encoded by one genetic locus, the ABO locus, which has three alternative forms: A, B, and O. A child receives one of these three forms from each parent, yielding six possible genotypes and four possible blood types (aka phenotypes: A, AB, B, O). ABO blood group antigens remain of prime importance in transfusion and transplantation medicine, and they are the most immunogenic of all the blood group antigens. ABO antibodies are of major clinical significance because they are naturally and universally occurring, and they are highly reactive. The distribution of the four phenotypes varies across the world - type O is the most common, followed by type A, then type B, and type AB is the least common. The ABO system is acknowledged in the pathogenesis of cardiovascular, infectious and neoplastic disorders, and pre-eclampsia. H antigen deficiency is known as the "Bombay phenotype" and is found in 1 in 10,000 people in India and 1 in 1 million people in Europe. [NCBI Books:NBK2267] PMID: 27177402[NCBI Books:NBK2264]

Rh is the most complex of the blood groups systems, encompassing over 45 distinct antigens. The most clinical important antigen, D or Rho, was first described in 1940 and is present in over 85% of the population. Those individuals who lack the D antigen are considered to be Rh negative.
The Rh antigens are encoded by two highly homologous and closely linked genes on the short arm of chromosome 1. The RHD gene codes for the D antigen, or most of its components; RhCE gene codes the Cc and Ee antigens or their variants. The majority of the antigens within this system represent products of gene cross-over, point mutations or deletions within one or both genes.
The Rh antigens appear to be expressed only on red blood cell and they appear early during red cell development. Antibodies against the Rh antigens have caused severe and fatal transfusion reactions and hemolytic disease of the newborn. Rh antigens have been implicated in many diseases such as auto-immune hemolytic anemia, auto-Rh antibodies. Collect: One 4 mL green (sodium or lithium heparin) or serum separator tube. Also acceptable: lavender or pink (EDTA).
