Vanillylmandelic acid (VMA) is the major metabolite of the catecholamines. Increased levels of VMA are often seen in patients with pheochromocytoma. VMA levels are of value for evaluation of hypertension, diagnosis and follow up of neuroblastoma, ganglioneuroma, and ganglioneuroblastoma. Urinary VMA is increased in about 90% of patients with neuroblastoma. Neuroblastoma patients with a Homovanillic acid (HVA) to VMA ratio of less than 1 or greater than 2 have an unfavorable prognosis. [Information from Tietz Clinical Guide to Laboratory Tests, 4th edition, AHB Wu, 2006 and LOINC partner 1, 2007 11 21.] Elevated VMA in cerebrospinal fluid (CSF) without a concomitant elevation of urine VMA may be a marker for neuroectodermal brain tumors and could potentially be used to monitor treatment efficacy. Creatinine or creatine anhydride, is a breakdown product of creatine phosphate in muscle. The loss of water molecule from creatine results in the formation of creatinine. It is transferred to the kidneys by blood plasma, whereupon it is eliminated by glomular filtration and partial tubular excretion. Creatinine is usually produced at a fairly constant rate and measuring its serum level is a simple test. A rise in blood creatinine levels is observed only with marked damage to functioning nephrons; therefore this test is not suitable for detecting early kidney disease. Creatine and creatinine are metabolized in the kidneys, muscle, liver and pancreas.
