LP14597-6 Platelets
Platelets are miniature fragments of cytoplasm that break off of megakaryocytes. Their primary function is to inhibit excessive hemorrhage after blood vessel injury. Platelets cling to the damaged part of the blood vessel where they become activated, recruit other platelets to the site, and begin the cascade of physical and chemical events that lead to clot formation. Activated platelets secrete cytokines, chemokines, and growth factors, all which play a role in thrombus development.When activated, platelets stimulate monocytes and neutrophils to leave the blood vessel and enter the tissue. PMID: 27226790 The normal range of platelets is 150,000 to 450,000 platelets/uL. Lower than normal platelet counts can lead to increased bleeding and is caused by a number of factors including: chemotherapy, autoimmune disorders and some medications. Increased platelets can cause hypercoagulation due to cancer, anemia, spleen removal and some medications.[MedlinePlus Encyclopedia:003647]

Anisocytosis indicates that red blood cells seen on blood smears vary in size. This phenomona occurs in anemia and other blood conditions. When measured via automatic machines, false diagnostic flagging may be triggered by an elevated WBC count, agglutinated RBCs, RBC fragments, giant platelets or platelet clumps. The red cell distribution width (RDW) is a measure of anisocytosis.

Basophilic stippling represents the spontaneous aggregation of ribosomal RNA in the cytoplasm of erythrocytes. The accumulation of these dots is pathologic and has been associated with sideroblastic anemia, lead poisoning, megaloblastic anemia and possibly beta thalassemia.

Cabot rings are thin, red-violet staining, threadlike strands in the shape of a loop or figure-8 that are found on rare occasions in erythrocytes. They are believed to be microtubules that are remnants from a mitotic spindle.
Cabot rings have been observed in a handful of cases in patients with megaloblastic anemia, lead poisoning and other disorders of erythropoiesis.

Helmet cells are helmet-shaped red blood cell fragments without a central pallor. Helmet cells are a subset of fragmented red cells, the other two being schistocytes and triangular cells. Fragmented red blood cells (rbcs) occur when the rbcs undergo rips and tears from fibrin strands in microcirculation or trauma occurring in macrocirculation. Howell-Jolly bodies are histopathological findings of basophilic nuclear remnants (clusters of DNA) in circulating erythrocytes. During maturation in the bone marrow, erythrocytes normally expel their nuclei, but in some cases, a small portion of DNA remains. Named for William Henry Howell and Justin Marie Jolly.

LP17441-4 Macrocytes
Macrocytosis is the enlargement of red blood cells with near-constant hemoglobin concentration, and is defined by a mean corpuscular volume (MCV) of greater than 100 femtolitres (the precise criterion varies between laboratories).
Most commonly (especially when the increase in size is mild, and just above normal range) the etiology is bone marrow dysplasia primary to alcohol abuse and Chronic alcoholism. Other causes may include: Megaloblastosis (Vitamin B12 or folate deficiency; or DNA synthesis-inhibiting drugs), hypothyroidism, reticulocytosis (commonly from hemolysis or a recent history of blood loss), liver disease, myeloproliferative disease

Pappenheimer bodies are abnormal granules of iron found inside red blood cells. They represent inclusion bodies in phagosomes that have engulfed excessive amounts of iron. They appear as one or two dense, blue-purple granules within the red blood cell, located in the cell periphery. They are detected in sideroblastic anemia, hemolytic anemia, and sickle cell disease. They can interfere with platelet counts when the analysis is performed by electro-optical counters.

LP30906-9 Poikilocytosis
Poikilocytosis are abnormally shaped red blood cells (RBC) sometimes seen on blood smears of humans and many wild and domestic animals. They are more common in some apparently normal small ruminants, particularly goats. Normal RBC are round, flattened disks that are thinner in the middle than at the edges, whereas a poikilocyte is either a distortion of that normal shape or an entirely different shape. Generally, poikilocytosis refers to an increase in abnormal RBC of any shape where they make up 10% or more of the total red cell population. Poikilocytosis is a rather general condition rather than a specific one.

Polychromasia occurs when there is an abnormally high number of red blood cells found in the bloodstream as a result of being prematurely released from the bone marrow. Polychromatophilic red cells are larger and younger than mature red cells and appear blue-gray on a blood smear. Polychromasia is usually a sign of bone marrow stress due to blood loss or other conditions.

Rouleaux (singular rouleau) are stacks of red blood cells (RBCs) which form because of the unique discoid shape of RBCs in vertebrates. The flat surface of the discoid RBCs provides a large surface area that allows the cells to stick to each other, thus forming rouleaux. Rouleaux occurs when the plasma protein concentration is high, leading to increased erythrocyte sedimentation rate. The presence of rouleaux does not always indicate the presence of a disease. Conditions which cause rouleaux formation include infections, inflammatory and connective tissue disorders, and cancers. It also occurs in diabetes mellitus and is one of the causes leading to microvascular occlusions in diabetic retinopathy.

A schistocyte or schizocyte is a fragmented part of a red blood cell (RBC). Schistocytes are typically irregularly shaped, jagged, and asymmetrical. A true schistocyte does not have central pallor.
Several microangiopathic diseases, including disseminated intravascular coagulation and thrombotic microangiopathies, generate fibrin strands that sever RBC as they try to move past a thrombus, creating schistocytes. Schistocytes are often seen in patients with hemolytic anemia and they frequently result from mechanical heart valve prostheses. Excessive schistocytes present in blood can be a sign of microangiopathic hemolytic anemia (MAHA) where the most common cause is aortic stenosis.

Sickle-cell anemia is an autosomal recessive genetic blood disorder characterized by the presence of rigid and sickle-shaped red blood cells. The sickling occurs because of a mutation in the hemoglubin gene. In the US, the prevalence of the disease is about 1 in 5000, mostly affecting Americans of Sub-Sahara African descent. One third of all inhabitants of Sub-Sahara Africa carry the gene because in areas where malaria is common there is a survival advantage in carrying only one single sickle-cell gene (sickle-cell trait).

Stomatocytes are red blood cells (rbcs) with an oval or rectangular area of central pallor, looking like a "mouth" on a stained blood smear differential. The indentation on one side of the red cell is missing. Stomatocytes usually have altered permeability and increased cell volume. Stomatocytes tend to get trapped in the spleen and other organs, resulting in hemolysis. 40-60% of rbcs may be stomatocytes in patients with hereditary stomatocytosis. Increased numbers of stomatocytes are seen with liver disease, electrolyte imbalance, and hereditary stomatocytosis. Stomatocytes may also be seen as an artifact on a drying blood smear, but they should only be seen on some areas on the periphery of the blood smear. Leukocytes or white blood cells (WBCs) are immune cells that fight infection, neoplasms and other inflammatory conditions, and mediate allergic responses. There are five types of WBCs normally present in the circulation that are all derived from a similar stem cell in the bone marrow. The five type of WBCs are divided into two groups based on the presence or absence of granules in the cytoplasm. The granulocytes include the neutrophils, basophils and eosinophils. The non-granulocytes include the lymphocytes and the monocytes. The neutrophils fight infection by ingesting and digesting bacteria. Eosinophils and basophils respond to allergic reactions and are capable of ingesting antigen-antibody complexes. Monocytes phagocytose bacteria and release interferon to stimulate the immune system. Lymphocytes are divided into T-cells and B-cells. T-cell immunity is cellular and involves the activation of phagocytes and B-cell immunity uses antibodies to fight infection. Both elevated and low leukocyte counts can be markers of infection and malignancy, and low leukocyte counts are associated with a variety of primary and secondary immunodeficiencies, depending on the WBC type(s) that are out of range. Auer rods are clumps of azurophilic granular material that form elongated needles and can be seen in the cytoplasm of leukemic blasts under microscopic examination. They are composed of fused lysosomes and contain peroxidase, lysosomal enzymes, and large crystalline inclusions.
Auer rods are classically seen in myeloid blasts of M1, M2, M3, and M4 acute leukemias.
They are also used to distinguish the pre-leukemia myelodysplastic syndromes: refractory anemia with excess blasts 2 (which has Auer rods) from RAEB 1 (which does not).

Not much is known about their formation, but döhle bodies are thought to be remnants of the rough endoplasmic reticulum. They are often present in conjunction with toxic granulation.

Hypersegmented neutrophils are neutrophils with 5 or more nuclear lobes. They are often seen in disorders of nuclear maturation and are thought to be pathognomonic of the class of anemias called megaloblastic anemias. Some studies, however, have strongly associated neutrophil hypersegmentation with iron deficiency anemia.
One of the earliest changes in the megaloblastic processes is the appearance of these hypersegmented neutrophils. Because of the short life-span of neutrophils, these abnormal cells characteristically appear even before the onset of anemia [2]. However, such neutrophils are not seen in the other classes of anemias, which incidently are far more common than megaloblastic anemias. It is of note that pernicious anemia is a type of megaloblastic anemia, and as such, may show hypersegemented neutrophils.

Leukocytes or white blood cells (WBCs) are immune cells that fight infection, neoplasms and other inflammatory conditions, and mediate allergic responses. There are five types of WBCs normally present in the circulation that are all derived from a similar stem cell in the bone marrow. The five type of WBCs are divided into two groups based on the presence or absence of granules in the cytoplasm. The granulocytes include the neutrophils, basophils and eosinophils. The non-granulocytes include the lymphocytes and the monocytes. The neutrophils fight infection by ingesting and digesting bacteria. Eosinophils and basophils respond to allergic reactions and are capable of ingesting antigen-antibody complexes. Monocytes phagocytose bacteria and release interferon to stimulate the immune system. Lymphocytes are divided into T-cells and B-cells. T-cell immunity is cellular and involves the activation of phagocytes and B-cell immunity uses antibodies to fight infection. Both elevated and low leukocyte counts can be markers of infection and malignancy, and low leukocyte counts are associated with a variety of primary and secondary immunodeficiencies, depending on the WBC type(s) that are out of range. Platelet satellitism occurs when platelets suround polymorphonuclear neutrophils (PMN) to form complex rosettes. This tends to occur at room temperature when EDTA is used as anticaogulant. IgG autoantibodies directed against the glycoprotein IIb/IIIa complex of the platelets and the Fc receptor III of neutrophils have been implicated in this phenomenon. The occurrence of platelet satellitism may cause a false decrease in platelet counts and it is important to identify it when reviewing differentials. Giant platelets are abnormally large platelets 7-15 microns in size. They are associated with inherited giant platelet disorders (IGPDs), a group of rare disorders characterized by thrombocytopenia, giant platelets, and a tendency to bleeding. IGPDs include Bernard-Soulier syndrome (BSS), velocardiofacial syndrome (VCFS), May-Hegglin anomaly (MHA), Epstein syndrome, and Mediterranean macrothrombocytopenia. PMID: 10664620
