The group of drugs called cannabinoids includes both the cannabinoid chemicals found in the cannabis plant as well as synthetic and pharmaceutical cannabinoids. The cannabinoids in cannabis include tetrahydrocannabinol (THC), cannabidiol (CBD), and cannabinol (CBN). THC is responsible for the psychoactive effects of cannabis and binds to the CB1 and CB2 cannabinoid receptors. CBD does not have psychoactive effects but rather has a variety of therapeutic properties including anticonvulsant, analgesic, anti-anxiety, and antiemetic. The THC to CBD ratio determines the physiologic effects of the drug. A high THC/CBD ratio results in euphoric and anxiolytic effects, while a low THC/CBD ratio results in sedative effects. Both natural and synthetic cannabinoid compounds are used for therapeutic purposes. Synthetic cannabinoids, like THC, act on the CB1 and CB2 receptors. Adverse reactions, including psychosis, confusion, agitation, tachycardia, myocardial infarction, seizures, and fatal overdoses, are more common with synthetic cannabinoids compared to cannabis. Confirmatory testing on oral fluid using liquid chromatography-mass spectrometry (LC-MS) is highly sensitive and specific even on relatively small sample sizes.
