17-Hydroxypregnenolone (also 17-OH-pregnenolone and 17-alpha-hydroxypregnenolone), is a C21 steroid that is obtained by hydroxylation of pregnenolone at the C17-alpha position. This step is performed by the mitochondrial cytochrome P450 enzyme 17-hydroxylase (CYP17A1) that is present in the adrenal and gonads. Peak levels are reached in humans at the end of puberty and then decline. High levels are also achieved during pregnancy.
Measurements of 17-OH-pregnenolone are useful in the diagnosis of certain forms of congenital adrenal hyperplasia. In patients with congenital adrenal hyperplasia due to 3 beta-hydroxysteroid dehydrogenase deficiency 17-OH-pregnenolone is increased, while in patients with congenital adrenal hyperplasia due to 17 alpha-hydroxylase deficiency levels are low to absent. 17-Hydroxypregnenolone, also known as 17-OH-PG, is a steroid hormone that is derived from cholesterol. 17-OH-PG is involved in the synthesis of cortisol, aldosterone and androstenedione in the adrenal glands. 17-OH-PG levels are measured to screen for congenital adrenal hyperplasia (CAH), a group of inherited disorders of the adrenal gland. Creatinine or creatine anhydride, is a breakdown product of creatine phosphate in muscle. The loss of water molecule from creatine results in the formation of creatinine. It is transferred to the kidneys by blood plasma, whereupon it is eliminated by glomular filtration and partial tubular excretion. Creatinine is usually produced at a fairly constant rate and measuring its serum level is a simple test. A rise in blood creatinine levels is observed only with marked damage to functioning nephrons; therefore this test is not suitable for detecting early kidney disease. Creatine and creatinine are metabolized in the kidneys, muscle, liver and pancreas.
