LP6300-0 Gradient strip
A gradient strip is a reagent strip that contains a predefined concentration gradient of a specific antibiotic or combination of antibiotics that is used for antimicrobial susceptibility testing (AST) and antimicrobial resistance detection (ARD).
For AST, the result is reported in terms of the minimum inhibitory concentration (MIC) of a specific antimicrobial that prevents the visible growth of bacteria. In LOINC, the "Gradient strip" method is used for AST gradient strips that result in an MIC result.
For ARD, the result is reported as the presence or absence of a specific enzyme or other mechanism that confers resistance, which is determined based on comparing the inhibitory concentration (IC) of a specific antimicrobial with the IC of that antimicrobial plus a resistance inhibitor. In LOINC, the "Gradient strip ARD" method is used for ARD gradient strips, which are not intended for determining individual MIC values.

Part Descriptions
LP15420-0 Amoxicillin+Clavulanate
The combination of Amoxicillin and clavulanate -- also known as Augmentin -- is an oral antibacterial combination consisting of the semisynthetic antibiotic amoxicillin and the -lactamase inhibitor, clavulanate . It is used in the treatment of otitis media, sinusitis, and infections caused by susceptible organisms involving the lower respiratory tract, skin and skin structure, and urinary tract; spectrum same as amoxicillin with additional coverage of beta-lactamase producing B. catarrhalis, H. influenzae, N. gonorrhoeae, and S. aureus (not MRSA). The expanded coverage of this combination makes it a useful alternative when penicillin resistance is present and patients cannot tolerate alternative treatments.

Ampicillin is a beta-lactam antibiotic that has been used extensively to treat bacterial infections since 1961, but because of resistance its usage has declined substantially It can sometimes result in allergic reactions that range in severity from a rash (i.e. patients with mononucleosis) to potentially lethal anaphylaxis.

6980-7 Ampicillin+Sulbactam [Susceptibility] by Gradient strip
Part Descriptions
LP15937-3 Ampicillin+Sulbactam
Ampicillin/sulbactam is a combination of the common penicillin-derived antibiotic ampicillin and sulbactam, an inhibitor of bacterial beta-lactamase. Two different forms of the drug exist. The first, developed in 1987 and marketed in the United States under the tradename Unasyn, is an intravenous antibiotic. The second, an oral form called sultamicillin, is marketed under the trade name Ampictam outside of the United States. Ampicillin/sulbactam is used to treat infections caused by bacteria resistant to beta-lactam antibiotics. Sulbactam blocks the enzyme which breaks down ampicillin and thereby allows ampicillin to attack and kill the bacteria.

6981-5 Azithromycin [Susceptibility] by Gradient strip
Part Descriptions
LP16090-0 Azithromycin
Azithromycin is an azalide, a subclass of macrolide antibiotics. Azithromycin (brand names Zithromax ® in Italy; Zmax ®; Sumamed ®; Aztrin ®) is one of the world's best-selling antibiotics, and is derived from erythromycin ; however, it differs chemically from erythromycin in that a methyl -substituted nitrogen atom is incorporated into the lactone ring, thus making the lactone ring 15-membered. Azithromycin is used to treat certain bacterial infections, most often bacteria causing middle ear infections, tonsillitis, throat infections, laryngitis, bronchitis, pneumonia and sinusitis. It is also effective against certain sexually transmitted infectious diseases, such as non-gonococcal urethritis and cervicitis. Recent studies have also shown it to be effective against late-onset asthma, but these findings are controversial and not widely accepted as of yet.

6982-3 Aztreonam [Susceptibility] by Gradient strip
Part Descriptions
LP16041-3 Aztreonam
Aztreonam (Azactam®) is a monocyclic beta-lactam antibiotic (a monobactam) originally isolated from Chromobacterium violaceum. It is resistant to some beta-lactamases, but is inactivated by extended-spectrum beta-lactamases. Aztreonam is effective against gram-negative bacteria, including Pseudomonas aeruginosa. It is not effective against gram-positive bacteria or anaerobes. Aztreonam has to be administered intravenously, as the compound is poorly absorbed when given orally. Side-effects include reactions at the injection site, diarrhea and other gastro-intestinal symptoms. There is limited cross-reactivity between aztreonam and other beta-lactam antibiotics, and it is generally considered safe to admininister aztreonam to patients allergic to penicillin.

LP16062-9 Carbenicillin
Carbenicillin is an antibiotic belonging to the carboxypenicillin subgroup of the penicillins. It has gram-negative coverage which includes Pseudomonas aeruginosa but limited gram-positive coverage. The carboxypenicillins are susceptible to degradation by beta-lactamase enzymes. The antibiotic is very soluble in water and is acid-labile. Aqueous solutions are short-lived. Working concentration in the lab: up to 100 µg per ml. It is a semi-synthetic analogue of the naturally occurring penicillin. Although much more expensive than ampicillin, it is much more stable.

55627-4 Cefacetrile [Susceptibility] by Gradient strip
Part Descriptions

LP14176-9 Cefaclor
Cefaclor is a semisynthetic β-lactam antibiotic drug belonging to the cephalosporin subgroup. Like all β-lactams, cefaclor readily reacts with tissue proteins in vivo, thereby sensitizing susceptible individuals. The allergenic determinants (epitopes) of cephalosporins such as cefaclor are not fully known. This is a consequence of the unstable structures which are formed upon reaction with lysine amino groups in proteins. The primary unstable conjugates are rapidly degraded and transformed into structures where the six-membered dihydrothiazine ring is absent.
Immediate reactions (maximum 1 hour after drug intake) have the highest probability of being IgE-mediated. Clinical pictures typical for immediate reactions to β-lactams are urticaria, with or without angioedema, and anaphylaxis.

41670-1 ceFAZolin [Susceptibility] by Gradient strip
Part Descriptions
LP16066-0 ceFAZolin
Cefazolin, also known as cefazoline or cephazolin, is a first generation cephalosporin antibiotic. The drug is usually administrated either by intramuscular injection (injection into a large muscle) or intravenous infusion (intravenous fluid into a vein). Cefazolin is mainly used to treat bacterial infections of the skin. It can also be used to treat moderately severe bacterial infections involving the lung, bone, joint, stomach, blood, heart valve, and urinary tract. It is effective only against infections caused by staphylococci and streptococci bacteria, organisms commonly found on normal human skin. Resistance to cefazolin is seen in several species of bacteria.

31143-1 Cefepime [Susceptibility] for meningitis by Gradient strip
Term Description
Susceptibility of an isolate to cefepime in the context of a patient with meningitis, i.e., the "meningitis breakpoint". Various agencies, including the FDA and CLSI in the U.S. and EUCAST in Europe, publish guidelines for determining susceptibility of different organisms to different antimicrobials. In most cases, susceptibility does not vary by type of infection, but for a few specific types of infection, including meningitis, pneumonia and urinary tract infection, certain antimicrobials have different susceptibility thresholds.

In most cases, susceptibility does not vary by infection site, but for a few specific types of infection, including meningitis, pneumonia and urinary tract infection, certain antimicrobials have different susceptibility thresholds, which are called "breakpoints". For terms in LOINC that represent specific breakpoints, the LOINC System will indicate the type of infection that the breakpoint is based on, for example, Isolate.meningitis, Isolate.pneumonia, and Isolate.UTI.

31139-9 Cefotaxime [Susceptibility] for meningitis by Gradient strip
Term Description
Susceptibility of an isolate to cefotaxime in the context of a patient with meningitis, i.e., the "meningitis breakpoint".

Part Descriptions

LP15210-5 Cefpodoxime
Cefpodoxime is a third generation beta-lactam antibiotic that belongs to the cephalosporin Family of antibiotics. Its mode of action is to bind to penicillin binding proteins (PBPs) that are located on the outer membrane of the bacterium. When cefpodoxime binds to the PBPs, it interferes with the activity of transpeptidase enzymes which prevents the formation of peptidoglycan chains. Without the peptidoglycan chains, the bacterial septum and cell wall cannot be produced during cell division, which in turn affects the rigidity of the cell wall and leads to cell lysis. Cefpodoxime is effective against both Gram-positive and Gram-negative bacteria and is used to treat acute tuberculosis, otitis media, pharyngitis, and sinusitis.[PubChem: 6335986]
