A high-molecular-weight proteoglycan-platelet factor complex which is released from blood platelets by thrombin. It acts as a mediator in the heparin-neutralizing capacity of the blood and plays a role in platelet aggregation. At high ionic strength (I=0.75), the complex dissociates into the active component (molecular weight 29,000) and the proteoglycan carrier (chondroitin 4-sulfate, molecular weight 350,000). The molecule exists in the form of a dimer consisting of 8 moles of platelet factor 4 and 2 moles of proteoglycan.

Platelet factor-4 (PF4) is a 70-amino acid protein that is released from the alpha-granules of activated platelets and binds with high affinity to heparin. Its major role is to neutralize heparin-like molecules on the endothelial surface of blood vessels, thereby inhibiting local antithrombin III activity and promoting coagulation. As a strong chemoattractant for neutrophils and fibroblasts, PF4 may play a role in inflammation and wound repair. The heparin:PF4 complex is the immunogen in heparin-induced thrombocytopenia, an idiosyncratic autoimmune reaction that sometimes occurs following the injection of heparin. PF4 autoantibodies have also been observed in patients with thrombosis and features resembling HIT although these patients did not receive heparin.
