Term Description
The Decayed, Missing, or Filled Tooth Surfaces (DMFS) Index is used by a dentist or dental hygienist to determine the prevalence of coronal caries in permanent and primary teeth through an examination that records the three components. Note from PhenX Oral Health Working Group (WG): The National Health and Nutrition Examination Survey (NHANES) uses a web-based system for data entry that will beep and a message will be displayed in the lower right portion of the screen if two entries are made or a nonallowable entry is made. Data entry is prohibited until an allowable response is entered. There are laptop programs that can be used with this protocol, and the Oral Health WG recommends such a system as it does notify the recorder of correctable errors. An example of a paper data recording form is included as part of this protocol so that the user can see the format of such a data entry system. It also is good to have paper forms on hand in case of an electrical failure or some other problem that prohibits use of the electronic data entry system.

Term Description
These interviewer-administered questions ask the subject or biological parents of a child with cleft lip or palate for a description of the orofacial defects present, the extent of the defect, and if another family member has been diagnosed with the same condition. This protocol is administered by an interviewer asking questions of a subject or of an informant (usually a parent) if the subject is a young child. Question 1 should be asked as written. If the answer is not "no," then the LAHSHAL (Lip, Alveolus, Hard palate, Soft palate, Hard palate, Alveolus, Lip) is completed to characterize the defect. Then the LAHSHAL is completed from the perspective of the interviewer looking at the subject (i.e., right to left). The "? - unknown" variable should only be filled in if the interviewer has exhausted all other means to obtain this data. For each of the five components, fill in a bubble next to appropriate code (ACIX?*) - key provided. Many cleft defects are evident to the interviewer, but still the subject or the informant should be asked about each part of the defect (lip, alveolus, hard palate, and soft palate). Record a cleft uvula as a microform cleft of the soft palate. A cleft in the alveolus in addition to cleft palate should be recorded as a "cleft lip and palate." Question 2 is also asked of each subject (or informant regarding the subject) even if the subject does not have an orofacial cleft.

Term Description
Angle's Classification is a set of criteria that categorizes the three different types of malocclusion. The interviewer-administered questions are used to determine if the participant has ever had orthodontic treatment. Note from PhenX Oral Health Working Group (WG): There are laptop programs that can be used with this protocol, and the Oral Health WG recommends such a system as it does notify the recorder of correctable errors. An example of a paper data recording form is included as part of this protocol so that the user can see the format of such a data entry system. It also is good to have paper forms on hand in case of an electrical failure or some other problem that prohibits use of the electronic data entry system.

Questions ask for self-reports about the number of natural teeth an adult or child has in his or her mouth. In addition, one question asks a parent or caregiver to record information on the number of teeth in his or her infant's mouth at 3-12 months of age. This is not a count of unnatural teeth; therefore, do not count implants and remove all false teeth, such as partial dentures and full dentures (some dentures are attached to implants and cannot be removed and should not be counted). If a bridge is present, count only the teeth remaining on top of which the bridge sits. For example, a three-unit bridge usually has two teeth or two abutments (one on each side) that should be included in the count and one tooth (in the middle) is usually missing. Note: This measure should only be used if the Dental Caries measure is not chosen. If the Dental Caries measure is chosen, this measure is redundant.

Term Description
This is a clinical examination performed by a dentist or dental hygienist that consists of probing up to 28 teeth at six sites per tooth with a periodontal probe to determine prevalence of periodontal disease. Note from PhenX Oral Health Working Group (WG): There are laptop programs that can be used with this protocol, and the Oral Health WG recommends such a system as it does notify the recorder of correctable errors. An example of a paper data recording form is included as part of this protocol so that the user can see the format of such a data entry system. It also is advisable to have paper forms on hand in case of an electrical failure or some other problem that prohibits use of the electronic data entry system. The periodontal assessment is performed only on participants without need for prophylactic antibiotics. Periodontal attachment levels and bleeding from probing are assessed on all teeth present, except for third molars. Only fully erupted permanent teeth are scored. Term Description
The Tooth Surface Index of Fluorosis (TSIF) assesses dental fluorosis for each tooth surface. Obtaining the Tooth Surface Index of Fluorosis (TSIF) requires a clinical examination by a trained dental professional, either a dentist or dental hygienist. Note from PhenX Oral Health Working Group (WG): There are laptop programs that can be used with this protocol, and the Oral Health WG recommends such a system as it does notify the recorder of correctable errors. An example of a paper data recording form is included as part of this protocol so that the user can see the format of such a data entry system. It also is advisable to have paper forms on hand in case of an electrical failure or some other problem that prohibits use of the electronic data entry system. The WG also recommends that when interested in the visible buccal/facial surfaces of the anterior teeth only, the lingual surfaces should not be scored.

The protective enamel covers the teeth and helps prevent cavities. Adult teeth naturally darken with age as the pulp within the tooth shrinks and dentin is deposited in its place. Teeth are among the most distinctive features of mammal species and fossils as well.
