Term Description

The health record cover sheet (aka "face sheet") is a document that contains summary patient identification and demographic information, provider identification, financial data, and some clinical information (e.g. admitting diagnosis, major procedures, admit/discharge dates etc). It is traditionally filed as the first page of a medical record.
