Epstein-Barr virus (EBV) is a gamma herpes virus also known as human herpes 4 and occurs only in humans. EBV is prevalent worldwide with over 90% of the adult population infected in their lifetime. The virus is spread mainly through body fluids and particularly through saliva. Symptoms of infection include fatigue, fever, inflamed throat, swollen lymph nodes in the neck, enlarged spleen, swollen liver and rash. EBV becomes dormant post infection but occasionally will reactivate especially in immunosuppressed patients. Laboratory tests are used to determine whether the infection is a primary infection (absence of anti-EBNA, presence of anti-VCA) or a past infection (presence of both anti-EBNA and anti-VCA) Epstein-Barr nuclear antigen begins to appear 2-4 months after symptoms first are evident and persist for the life of the patient. (http://www.cdc.gov/epstein-barr/laboratory-testing.html) Epstein-Barr nuclear antigen 1 (EBNA1) antibody is the main antibody tested in immunoassay tests. It is also the only EBV protein regularly present in EBV-associate neoplasms. There is little difference between EBNA 1 and EBNA 2 other than in their prevalence across the globe. Viral capsid antigen (VCA) p150 antibody is most commonly used in laboratory testing, but VCA p18 is also utilized though some reports of mixed results has been observed.(Manual of Clinical Microbiology, 11th edition, James H.
