Part Descriptions
LP16048-8 Cocaine
Cocaine is an alkaloid of Erythroxylon coca that is grown in the northern South American Andes, India, Africa and Java. It is used medicinally as a local anesthetic and vasoconstrictor in nasal surgery and to dilate pupils. Because cocaine is a powerful central nervous system stimulant it is among the most abused drugs. Cocaine abuse can lead to sudden death due to cardiotoxicity or the sequential development of hyperthermia, agitated delirium and respiratory arrest. Cocaine is metabolized in the liver to inactive benzoylecgonine (BZE). BZE is pharmacologically inactive and has a longer half-life than its parent drug cocaine. It is the substance most tested for when investigating suspected cocaine abuse. Cocaethylene (CE) is produced in the liver in the presence of ethanol and has been shown to have similar stimulatory effects as cocaine in animal studies.(Tietz Fundamentals of Clinical Chemistry and Molecular Diagnostics, Seventh Edition) Cocaethylene is toxic to the myocardium. LP16048-8 Cocaine
An alkaloid ester extracted from the leaves of plants including coca. It is a local anesthetic and vasoconstrictor and is clinically used for that purpose, particularly in the eye, ear, nose, and throat. It also has powerful central nervous system effects similar to the amphetamines and is a drug of abuse. Cocaine, like amphetamines, acts by multiple mechanisms on brain catecholaminergic neurons; the mechanism of its reinforcing effects is thought to involve inhibition of dopamine uptake.

69010-7 Benzoylecgonine [Mass/mass] in Meconium by Confirmatory method
Part Descriptions
LP16047-0 Benzoylecgonine
Benzoylecgonine is the major metabolite of cocaine. It is a biological metabolite with a stable structure that resists hydrolysis, both in the body and in the environment. Benzoylecgonine remains in the body for a few days and is commonly used as a marker in drug tests. [PMCID:1929267] PMID: 27224254

69011-5 Cocaethylene [Mass/mass] in Meconium by Confirmatory method
Part Descriptions
LP18505-5 Cocaethylene
Cocaethylene is the ethyl ester of benzoylecgonine. It is chemically related to cocaine, which is the corresponding methyl ester. Cocaethylene is formed in the body when cocaine and alcohol have been taken simultaneously: the transesterification is catalysed by carboxylesterases in the liver. Cocaethylene has similar euphoric properties to cocaine, but is longer lasting in the body. Some studies suggest that it is more toxic than cocaine itself, particularly to the heart. PMID: 12485948

- Change Reason
- The Property has been changed from ACnc to reflect new model for ordinal terms; The PrThr property is used for LOINC terms whose results are reported using an ordered categorical scale, regardless of whether or not an internal threshold was used to make that determination. This change was approved by the Laboratory LOINC Committee in June 2016.
