This term was created for (but not limited to) Siemen's Immulite 2000 AlaTOP allergy screen, a chemiluminescent enzyme-labeled sequential immunoassay used to qualitatively detect IgE antibodies specific to inhalant allergens in serum. The AlaTOP constituents usually include the following 12 allergens: Dermatophagoides pteronyssinus (dust mite), cat epithelium, dog dander, Cynodon dactylon (Bermuda grass), Phleum pretense (timothy grass), Penicillium notatum, Alternaria tenuis, Ambrosia artemisiifolia (common ragweed), Plantago lanceolata (English plantain), Parietaria officinalis (wall pelitory), Betula papyrifera (paper birch), and Cryptomeria japonica (Japanese cedar) (Source: http://www.cdc.gov/niosh/hhe/reports/pdfs/2005-0248-3077.pdf) The Phadiatop assay has a fixed set of allergens that is not stated by the manufacturer (http://www.jiaci.org/issues/vol15issue02/7.pdf), but probably contains a similar subset of components. Test results are determined by the calculation of a signal to cutoff (S/CO) ratio, which is calculated automatically by the instrument. The results are reported as indeterminate (ratio between 0.9 and <1.1), reactive (ratio of >=1.1) and nonreactive (ratio of < 0.9).

Multiple inhalant allergen screening tests, such as the ImmunoCap Phadiatop and Siemen's IMMULITE Alatop, are used to assess if an inhalant allergy is present in conditions such as asthma and rhinitis. Each screening test has extracts from various allergens and can be adapted to include locally implicated aero-allergens. A screening test doesn't indicate which individual allergens are implicated but rather whether there is respiratory allergy or not. Immunoglobulin E (IgE) is the antibody found in blood and tissues which causes allergies. Elevated levels of IgE suggest an inhalant allergy is present. Source: Regenstrief LOINC

Change Reason
The PrThr property is used for LOINC terms whose results are reported using an ordered categorical scale, regardless of whether or not an internal threshold was used to make that determination. This change was approved by the Laboratory LOINC Committee in June 2016.; The EIA method, which was always intended to cover more than just enzyme-linked immunoassay and whose display name for the long common name has always been Immunoassay, was renamed IA in order to eliminate ambiguity about whether the method has a broader meaning than just enzyme-linked immunoassay. Likewise, EIA.rapid was renamed IA.rapid. These changes were approved by the Laboratory LOINC Committee in June 2016.
