Normally bilirubin is thought to enter CSF only by passing through the blood brain barrier bound to albumin. This allows us to predict an "expected" CSF bilirubin concentration. Since bilirubin is a breakdown product of oxyhemoglobin, the bilirubin excess in CSF can be used to predict subarachnoid hemorrhage in the absence of cranial CT findings. Billirubin excess is defined as the surplus CSF bilirubin concentration measured after subtraction of an estimated upper limit for the individual patient. This upper limit, called the ''CSF bilirubin limit'' (CBL), is a calculated by multiplying the serum bilirubin concentration times the albumin quotient: ([bilirubin]serum X [albumin]CSF/[albumin]serum) X 1.25." Methemoglobin is a form of the oxygen-carrying protein hemoglobin, in which the iron in the heme group is in the Fe3+ state, not the Fe2+ of normal hemoglobin. Metheomglobin appears blue and is unable to carry oxygen. The NADH-dependent enzyme methemoglobin reductase (AKA diaphorase I) is responsible for converting methemoglobin back to hemoglobin. Normally 1-2% of people's hemoglobin is methemoglobin, a higher percentage than this can be genetic or caused by exposure to various chemicals and depending on the level can cause health problems. A higher level of methemoglobin will tend to cause a pulse oximeter to read closer to 85% regardless of the true level of oxygen saturation. Methemoglobinemia can be caused by genetic abnormalities such as diaphorase I deficiency, or abnormal hemoglobin variants. It can also be caused by certain oxidizing drugs (benzocaine), antibiotics (trimethoprim or sulfanamides), analine dyes, metoclopramide, chlorates, bromates, and nitrates. Infants under 6 months are particularly susceptible when exposed to nitrates, benzocaine, or even dehydration. Methemoglobinemia can be treated with supplemental oxygen and methylene blue.

Oxyhemoglobin is the oxygen-loaded form of hemoglobin, the transport molecule for oxygen in the blood. Each hemoglobin molecule is a tetramer with 4 sites that reversibly bind oxygen. Loading occurs in the lung where the pO2 is high, and unloading occurs in tissues according to the hemoglobin saturation curve. The fraction of hemoglobin sites loaded with an oxygen molecule is close to 100% in arterial blood and 60-80% in mixed venous blood. Changes in absorption spectra of hemoglobin upon binding oxygen is the basis for pulse oximetry, and partly account for its bright red color compared to the purple blue of deoxyhemoglobin due to a higher absorption spectrum (940 nm vs. 660 nm wavelength for oxyhemoglobin).

Xanthochromia refers to the yellowish appearance of cerebrospinal fluid in certain medical conditions, especially subarachnoid hemorrhage, when red blood cells (RBC) enter the CSF during bleeding. RBCs are eventually destroyed, releasing the oxygen-carrying molecule heme, which in turn is degraded to yield bilirubin, a yellow-green pigment. Heme that enters the CSF for different reasons (for example when a small blood vessel is damaged during a lumbar puncture), has no time to be digested and bilirubin is not generated. Many laboratories rely on the color of the fluid alone when testing for xanthochromia. Recent guidelines, however, suggest that spectrophotometry should be performed since bilirubin absorbs light of wavelengths between 450-460 nm. Two related substances, oxyhemoglobin and methemoglobin (410-418 nm and 403-410 nm absorption ranges, respectively) that are also released when heme is metabolized may be detected during this process. There are other causes for xanthochromia.
