Creatinine or creatine anhydride, is a breakdown product of creatine phosphate in muscle. The loss of water molecule from creatine results in the formation of creatinine. It is transferred to the kidneys by blood plasma, whereupon it is eliminated by glomular filtration and partial tubular excretion. Creatinine is usually produced at a fairly constant rate and measuring its serum level is a simple test. A rise in blood creatinine levels is observed only with marked damage to functioning nephrons; therefore this test is not suitable for detecting early kidney disease. Creatine and creatinine are metabolized in the kidneys, muscle, liver and pancreas.

5-hydroxyindoleacetate (5HIAA) is a metabolite of serotonin and is excreted in the urine. Serotonin is produced in large amounts by carcinoid tumors. Carcinoid tumors are rare, slow-growing tumors usually found in the lining of the digestive tract or in the lungs. The serotonin that is released by the carcinoid tumors is metabolized by monoamine oxidase in the liver, lungs, and brain to 5HIAA. Testing for elevated levels of 5HIAA in 24 hour urine specimens is 73% sensitive and 100% specific for the diagnosis of carcinoid tumors. Creatinine or creatine anhydride, is a breakdown product of creatine phosphate in muscle. The loss of water molecule from creatine results in the formation of creatinine. It is transferred to the kidneys by blood plasma, whereupon it is eliminated by glomular filtration and partial tubular excretion. Creatinine is usually produced at a fairly constant rate and measuring its serum level is a simple test. A rise in blood creatinine levels is observed only with marked damage to functioning nephrons; therefore this test is not suitable for detecting early kidney disease. Creatine and creatinine are metabolized in the kidneys, muscle, liver and pancreas.

3,4-Dihydroxyphenylacetate (DOPAC) is a metabolite of dopamine via the monoamine oxidase (MAO) enzyme. Measurement of DOPAC in conjunction with dopamine can be used to examine dopamine turnover and MAO activity. There is also mention in the literature of using it in the evaluation of neuroblastoma, as well as in studying renal dopaminergic processing.

Vanillylmandelic acid (VMA) is the major metabolite of the catecholamines. Increased levels of VMA are often seen in patients with pheochromocytoma. VMA levels are of value for evaluation of hypertension, diagnosis and follow up of neuroblastoma, ganglioneuroma, and ganglioneuroblastoma. Urinary VMA is increased in about 90% of patients with neuroblastoma. Neuroblastoma patients with a Homovanillic acid (HVA) to VMA ratio of less than 1 or greater than 2 have an unfavorable prognosis. [Information from Tietz Clinical Guide to Laboratory Tests, 4th edition, AHB Wu, 2006 and LOINC partner 1, 2007 11 21.] Elevated VMA in cerebrospinal fluid (CSF) without a concomitant elevation of urine VMA may be a marker for neuroectodermal brain tumors and could potentially be used to monitor treatment efficacy.
