Term Description
CYP3A4 gene mutation analysis for targeted polymorphisms (SNPs) is performed to determine a patient's allellic genotype and overall drug metabolism phenotype (ultrarapid, extenisive, intermediate, or poor metabolizer). This term is used to report the patient's allelic genotype, drug metabolism phenotype and overall interpretation in a single narrative report. The submitter's lab currently (as of 2013) tests for the following CYP3A4 alleles: *1B, *1G, *3, *4, *5, *6, *10, *12, *17, *18, *20 and *22. Selection is based on current literature. Cytochrome P450 3A4 (abbreviated CYP3A4) is part of a cluster of cytochrome P450 genes on chromosome 7q21.1. Over 30 single nucleotide polymorphisms (SNPs) have been identified in this gene, most of which are not clinically significant. Few have been found to decrease catalytic activity with certain ligands, including testosterone and nifedipine, in comparison to wild-type metabolism. Examples of these alleles and associated SNPs include CYP3A4*6 (an A17776 insertion), CYP3A4*17 (15615T>C) and CYP3A4*22 (15389C>T). The CYP3A4 gene (cytochrome P450, family 3, subfamily A, polypeptide 4) [HGNC Gene ID:2637] is located on chromosome 7q21.1. This gene encodes a member of the cytochrome P450 superfamily of enzymes. The cytochrome P450 proteins are monooxygenases that catalyze many reactions involved in drug metabolism and synthesis of cholesterol, steroids and other lipids. This protein localizes to the endoplasmic reticulum and its expression is induced by glucocorticoids and some pharmacological agents. This enzyme is involved in the metabolism of approximately half the drugs in use today, including acetaminophen, codeine, cyclosporin A, diazepam and erythromycin. The enzyme also metabolizes some steroids and carcinogens. This gene is part of a cluster of cytochrome P450 genes on chromosome 7q21.1. Previously another CYP3A gene, CYP3A3, was thought to exist; however, it is now thought that this sequence represents a transcript variant of CYP3A4.
