Tacrolimus (also known as FK-506 or Prograf) is a macrolide isolated from the culture broth of a strain of Streptomyces tsukubaensis. It has strong immunosuppressive properties and is commonly used to prevent allograft rejection in transplant recipients. One of its modes of action is to prevent T-lymphocyte activation.

Tacrolimus (also called FK-506 or fujimycin, Prograf, Advagraf, or Protopic) is amacrolide lactone, calcineurin inhibitor immunosuppresent used mainly after allogeneic organ transplant to lower the risk of organ rejection. It possesses strong immunosuppressive activity by preventing the activation by of T-lymphocytes in response to antigenic or mitogenic stimulation. Tacrolimus is also used in a topical preparation in the treatment of atopic dermatitis (eczema), severe refractory uveitis after bone marrow transplants, exacerbations of minimal change disease, TH2-mediated diseases such as Kimura's disease, and the skin condition vitiligo. There are potentially severe side effects from doses that are either too low or too high; inadequate tacrolimus concentrations may result in rejection of the transplanted organ, while high levels may lead to severe adverse effects includin nephrotoxicity, neurotoxicity, gastrointestinal disturbances, diabetogenesis, hypertension and malignant complications.
