The human soluble transferrin receptor (sTfR)/log ferritin index is intended as an aid in the diagnosis of iron deficiency anemia (IDA), and for the differential diagnosis of IDA and anemia of chronic disease (ACD). This term was created for but not limited in use to, the Coulter Soluble Transferrin Receptor sTfR /log Ferritin Index via automated chemiluminescent immunoassay.

Soluble transferrin receptor (sTfR) is a single polypeptide chain that is derived from transferrin receptor. The transferring receptor is mostly expressed on cells that require iron. sTfR concentration in blood correlates with the density of transferrin receptors on red blood cells (RBCs) and the number of RBCs with transferrin receptors being expressed, which closely correlates to iron requirements and RBC proliferation. sTfR has been proposed as an alternative marker for anemia of chronic disease (ACD), iron-deficiency anemia (IDA), and mixed ACD+IDA rather than the conventional tests, ferritin and transferrin. Ferritin and transferrin are both acute-phase reactants and are greatly affected by inflammation, infection and malignancy. sTfR is not affected by chronic disease or inflammation. sTfR can distinguish between ADC and IDA because sTfR increases in IDA but not in ACD.
