The ERBB2 gene (erb-b2 receptor tyrosine kinase 2) [HGNC Gene ID:3430] is located on chromosome 17q12. This gene encodes a member of the epidermal growth factor (EGF) receptor family of receptor tyrosine kinases. This protein has no ligand binding domain of its own and therefore cannot bind growth factors. However, it does bind tightly to other ligand-bound EGF receptor family members to form a heterodimer, stabilizing ligand binding and enhancing kinase-mediated activation of downstream signalling pathways, such as those involving mitogen-activated protein kinase and phosphatidylinositol-3 kinase. Allelic variations at amino acid positions 654 and 655 of isoform a (positions 624 and 625 of isoform b) have been reported, with the most common allele, Ile654/Ile655, shown here. Amplification and/or overexpression of this gene has been reported in numerous cancers, including breast and ovarian tumors. Alternative splicing results in several additional transcript variants, some encoding different isoforms and others that have not been fully characterized. Term Description
The ratio of HER-2/neu (ERBB2 gene) to CEP17 probes by in situ hybridization assays (e.g. FISH) to determine overexpression of HER-2/neu protein in breast cancer. CEP17 is used as an internal control and to account for aneusomy (varying number) of chromosome 17. A ratio of >2.0 indicates gene amplification and overexpression of HER-2/neu according to the 2013 ASCO-CAP HER2 test guideline recommendations. Results are used to assist in dose selection for certain drugs, assess eligibility for trastuzumab (Herceptin®) treatment, predict response to drug therapies and overall survival in patients with breast cancer. The average number of fluorescent signals targeting the ERBB2 gene (HER2 locus) in nuclei are determined in order to calculate the average ratio of ERBB2 gene to Chromosome 17 copy number [LOINC: 49683-6]. This term is based on, but not limited in use to, the PathVysion HER-2 DNA Probe Kit, which detects amplification of the ERBB2 gene via fluorescence in situ hybridization (FISH) in formalin-fixed, paraffin-embedded human breast and gastric cancer tissue specimens. The kit contains two probes: one specific to the HER-2/neu gene locus (ERBB2 gene, 17q11.2-q12) and another targeting the alpha satellite DNA sequence at the centromeric region of chromosome 17 (17p11.1-q11.1), called CEP17 (Chromosome Enumeration Probe 17). The average number of HER2 signals along with CEP17 signals [LOINC: 74861-6] are determined by microscopic examination within the same set of nuclei. The specimen is positive for ERBB2 gene amplification if the mean ratio of HER2 to CEP17 signals per nucleus is greater than or equal to 2.00.
