Acetylcholinesterase, an enyzme (EC 3.1.1.7) (AChE) that controls synaptic and neurohumoral cholinergic activity by hydrolyzing the neurotransmitter acetylcholine, is also known as RBC cholinesterase, erythrocyte cholinesterase, or (most formally) acetylcholine acetylhydrolase. The ACHE gene is mapped to chromosome 7q22. AChE is found primarily in the blood on red blood cell membranes, in neuromuscular junctions, and in neural synapses. Two main clinical diagnostic uses are: 1) a second trimester screening test for fetal neural tube defects using amniotic fluid and, 2) in the general population, to detect past exposure to organophosphate insecticides with resultant inhibition of the enzyme using blood.

Term Description
An interpretation of the acetylcholinesterase (AChE) in amniotic fluid testing result which is used as an aid to diagnose open neural tube defect (NDT) and ventral wall defects. The two most common neural tube defects are spina bifida and anencephaly. When an NTD is suspected based upon maternal serum alpha-fetoprotein (AFP) screening results, or diagnosed via ultrasound, detection of acetylcholinesterase (AChE) in amniotic fluid is a useful diagnostic tool as normal amniotic fluid does not contain AChE, unless from the fetus as a result of an open NTD. The patient's gestational age must be at least 13 weeks (second trimester) for accurate AChE detection. Optimal gestational age is 14 to 18 weeks. If acetylcholinesterase is present genetic consultation and high-resolution ultrasound may be suggested.

Hemoglobin F is used to screen for fetal-to-maternal hemorrrhage. The presence of fetal red cells in the maternal circulation may be identified by use of the acid elution principle first described by Kleihauer, Brown, and Betke, or by several modifications. Very small volumes of red cells commonly escape from the fetal intravascular compartment across the placental barrier into the maternal intervillous space.
