Phenobarbital is an antiepileptic drug first introduced for clinical use in 1912. It is still used to treat most seizures except for absence seizure types. The CYP2C19 enzyme metabolizes phenobarbital to p-hydroxyphenobarbital which is then excreted as glucuronide. Phenobarbital induces secretion of liver enzymes which can impact the efficacy of drugs administered at the same time. The most common side effect of phenobarbital in adults with blood levels greater than 40 mg/L is sedation. Primidone is an anticonvulsant of the pyrimidinedione[4] class, the active metabolites of which, phenobarbital (major) and phenylethylmalonamide (PEMA) (minor), are also anticonvulsants. It is used mainly to treat complex partial, simple partial, generalized tonic-clonic seizures, myoclonic, and akinetic seizures and has been a valuable alternative to propranolol in the treatment of essential tremor. Unlike other anticonvulsants such as carbamazepine and valproic acid, primidone is rarely used in the treatment of bipolar disorder or any other psychiatric problem. It is also not widely used in treatment of neuropathic pain, trigeminal neuralgia, migraine. Primidone has been occasionally used to treat long QT syndrome, cerebral palsy, and athetosis.
