Testosterone is an androgenic steroid and is the primary male sex hormone. Approximately half of circulating testosterone is tightly bound to a protein called sex hormone binding globulin (SHBG) and is biologically inactive. Another fraction is weakly bound to other proteins, such as albumin, and the remainder is unbound, or "free". Weakly bound and free testosterone together represent bioavailable testosterone. Both total and free testosterone levels decrease with age. Low testosterone is associated with many other conditions, including osteoporosis, Alzheimer's disease, obesity, diabetes and ischemic heart disease. PMID: 18488876

A potent androgenic steroid and major product secreted by the Leydig cells of the testis. Its production is stimulated by luteinizing hormone from the pituitary gland. In turn, testosterone exerts feedback control of the pituitary LH and FSH secretion. Depending on the tissues, testosterone can be further converted to dihydrotestosterone or estradiol.

Cortisol is a glucocorticoid produced in the adrenal cortex. A synthetic version of this same chemical is called hydrocortisone and manufactured as medication. Cortisol is not to be confused with cortisone, a closely related compound with much less (PubChem calls it "inactive") biologic activity. Cortisone is also offered as a drug, and it is converted to cortisol (the active form) in the liver. They both have a circadian rhythm with peak in the AM (around 8AM) and trough in the evening (around midnight), but this shifts according to sleep habits (work shifts). The morning measurement, which should be high, is most useful for diagnosing Addison's disease (too little cortisol) and the evening measurement, which should be low, is the most useful for diagnosing Cushing's disease (too much cortisol). Salivary cortisol may be a more sensitive measure than serum or plasma.

Androstenedione (also known as 4-androstenedione) is a 19-carbon steroid hormone produced in the adrenal glands and the gonads as an intermediate step in the biochemical pathway that produces the androgen testosterone and the estrogens estrone and estradiol. It is the common precursor of male and female sex hormones. Some androstenedione is also secreted into the plasma, and may be converted in peripheral tissues to testosterone and estrogens. Androstenedione originates either from the conversion of dehydroepiandrosterone or from 17-hydroxyprogesterone. It is further converted to either testosterone or estrone. The production of adrenal androstenedione is governed by ACTH, while production of gonadal androstenedione is controled by gonadotropins. In premenopausal women the adrenal glands and ovaries each produce about half of the total androstendione (about 3 mgday).

17-Hydroxypregnenolone (also 17-OH-pregnenolone and 17-alpha-hydroxypregnenolone), is a C21 steroid that is obtained by hydroxylation of pregnenolone at the C17-alpha position. This step is performed by the mitochondrial cytochrome P450 enzyme 17-hydroxylase (CYP17A1) that is present in the adrenal and gonads. Peak levels are reached in humans at the end of puberty and then decline. High levels are also achieved during pregnancy.
Measurements of 17-OH-pregnenolone are useful in the diagnosis of certain forms of congenital adrenal hyperplasia. In patients with congenital adrenal hyperplasia due to 3 beta-hydroxysteroid dehydrogenase deficiency 17-OH-pregnenolone is increased, while in patients with congenital adrenal hyperplasia due to 17 alpha-hydroxylase deficiency levels are low to absent. Measurements of 17-OH-pregnenolone are useful in the diagnosis of certain forms of congenital adrenal hyperplasia. Aldosterone is a steroid hormone produced by the outer-section (zona glomerulosa) of the adrenal cortex in the adrenal gland to regulate sodium and potassium balance in the blood. It is synthesized from cholesterol by aldosterone synthase, which is absent in other sections of the adrenal gland. It is the sole endogenous member of the class of mineralocorticoids. Acting on mineralocorticoid receptors (MR) on principal cells in the collecting ducts of the kidneys, it increases the permeability of their apical (luminal) membrane to potassium and sodium and activates their basolateral Na+K+ pumps, stimulating ATP hydrolysis, reabsorbing sodium (Na+) ions and water into the blood, and excreting potassium (K+) ions into the urine. Aldosterone also stimulates H+ secretion by intercalated cells in the collecting duct, regulating plasma bicarbonate (HCO3-) levels and its acidbase balance.

Used to detect hyper and hypo aldosteronisnm. Hypoaldosteronism can occur in any condition that causes destruction or dysfunction of the adrenal gland.1,2,4 Hypoaldosterism can be due to primary adrenal insufficiency , congenital adrenal hypoplasia, acquired secondary aldosterone deficiency (hyporeninemic hypoaldosteronism), and acquired Hyporeninemic hypoaldosteronism is the most common form of isolated hypoaldosteronism and is results from impaired renin release from the kidney. Congenital hypoaldosteronism caused by inherited enzymatic defects in aldosterone biosynthesis are rare. Corticosterone methyloxidase I (CMO I) deficiency is associated with elevated serum levels of corticosterone and low levels of 18-hydroxy-corticosterone and aldosterone. Corticosterone methyloxidase II (CMO II) deficiency produces high levels of 18-hydroxy-corticosterone, the immediate precursor of aldosterone.
Acquired primary hypoaldosteronism can be caused by the administration of heparin. Also, persistently hypotensive, critically ill patients with sepsis, pneumonia, peritonitis, cholangitis, and liver failure can have inappropriately low plasma aldosterone concentrations in relation to elevated plasma renin activity. Primary hyperaldosteronism, also referred to as Conn syndrome, is due to overproduction of aldosterone by one or both of the adrenal glands. Some recent studies suggest that 10% to 15% of Hyper thyroidism cases are due to (or associated with ) primary hyperaldosteronism. Secondary hyperaldosteronism is relatively common and can occur as the result of any condition that decreases blood flow to the kidneys (ie, renal artery stenosis), decreases blood pressure, or lowers plasma sodium levels. Secondary hyperaldosteronism may also be seen with cirrhosis, congestive heart failure. and toxemia of pregnancy. Several studies have suggested that high-normal aldosterone levels predict development of high blood pressure in normotensive subjects and that increased aldosterone action contributes to hypertension, cardiovascular fibrosis, and cardiac hypertrophy.

17-hydroxyprogesterone (17-OHP) is a steroid that is a member of the adrenal biosynthetic pathway and measured in order to diagnose congenital adrenal hyperplasia (CAH). Classic CAH is suspected when virilisation is seen in females at birth. The diagnosis of CAH in boys can be delayed. CAH is characterized by decreased cortisol as a result of a mutation in the CYP21A2 gene, which encodes the enzyme 21-hydroxylase. Increased blood concentrations of 17-OHP indicate a deficiency in 21-hydroxylase since 17-OHP is the substrate for 21-hydroxylase. Reduced levels of 21-hydroxylase negatively affect the production of cortisol. CAH is among the most frequent genetic disorders in humans. The severity of the physical manifestations are related to the degree of 21-hydroxylase deficiency. PMID: 24711560
