Creatinine or creatine anhydride, is a breakdown product of creatine phosphate in muscle. The loss of water molecule from creatine results in the formation of creatinine. It is transferred to the kidneys by blood plasma, whereupon it is eliminated by glomular filtration and partial tubular excretion. Creatinine is usually produced at a fairly constant rate and measuring its serum level is a simple test. A rise in blood creatinine levels is observed only with marked damage to functioning nephrons; therefore this test is not suitable for detecting early kidney disease. Creatine and creatinine are metabolized in the kidneys, muscle, liver and pancreas.

7-Isobiopterin (also known as 7-biopterin or primapterin) [PubChem: 5488923], is a pterin derivative of 4a-hydroxy-BH4. In healthy patients, tetrahydrobiopterin (BH4) is converted to 4a-hydroxy-BH4 with a concomitant conversion of phenylalanine to tyrosine, and BH4 is subsequently regenerated via a pathway that includes pterin-4a-carbinolamine dehydratase (PCD), which is encoded by the PCBD gene. Mutations in the PCBD gene are associated with decreased PCD activity, which results in decreased BH4 salvage, increase in phenylalanine due to inhibition of the BH4 to 4a-hydroxy-BH4 reaction, and concomitant increase in 7-biopterin in urine due to alternate metabolism of 4a-hydroxy-BH4. Biopterin is a coenzyme that is produced within the body. It is an oxidized degradation product of tetrahydrobiopterin. Defects in biopterin synthesis or regeneration can cause a form of hyperphenylalaninemia (a disease with symptoms similar to PKU) that is associated with a decrease in urine biopterin and a relative increase in urine neopterin. Biopterin is synthesized in several parts of the body, including the pineal gland. Biopterin deficiency has been associated with a variety of disorders, including dystonia and rabies.
