A single-item question used to determine the patient's overall financial resource strain. Patients are asked to select one answer (very hard, hard, somewhat hard, or not very hard). This single question has been found to be a valid measure of general financial resource strain. PMID: 23969421 ( PMID: 19189781 Evidence from the CARDIA study demonstrated the value of measuring the difficulty of paying for basics over time (e.g., financial resource strain), because there appeared to be a cumulative effect (e.g., incident hypertension).

Term Description
A single-question stress measure converged with items on psychological symptoms and sleep disturbances and with validated measures of well-being scored on a five-point Likert scale. PMID: 14712852 The question reads as follows:
Stress means a situation in which a person feels tense, restless, nervous, or anxious, or is unable to sleep at night because his/her mind is troubled all the time. Do you feel this kind of stress these days?

1 = "not at all," 2 = "only a little," 3 = "to some extent," 4 = "rather much," and 5 = "very much"
This question has been primarily tested in Scandinavian populations and can be linked to the PROMIS Emotional Distress (Depression and Anxiety) Short Form scales, which also are reliable and valid ways to assess stress. As of 2015, there is no clinical cutoff for determining when interventions, such as referral to stress management, are warranted.

Term Description
The Alcohol Use Disorders Identification Test C (AUDIT-C) is scored on a scale of 0-12 where the higher the score, the more likely the patient's drinking is hazardous. A score of 4 or more for men and 3 or more for women is considered positive for hazardous drinking or active alcohol use disorders. If the points are all from Question 1 alone where 2 and 3 are 0, it is likely the patient is drinking below recommended limits. The care provider may review the patients alcohol intake over that past few months to confirm accuracy. PMID: 12695273

Term Description
The Social connection and isolation panel is a set of five questions used to assess the number of types of social relationships on which a patient is connected and not isolated. Individuals receive one point for each of the following: being married or living together with someone in a partnership at the time of questioning, averaging three or more social interactions per week, attending church or other religious services more than four times per year, and reporting that they belong to a club or organization. A score of 0 (a summation of social isolation risks) represents the highest level of social isolation and a score of 4 represents the lowest level of social isolation. Among both men and women, a low social isolation score was associated with a mortality hazard ratio similar to that related to smoking and greater than that related to the other traditional clinical risk factors. PMID: 24028260 This study used the original NHANES III marital status question with an answer list that included 'Married - spouse in household' and 'Married - spouse not in household'. This panel includes the more common and recent marital status term since the answer list is more representative of what is being asked today.
