Medium chain triglycerides (MCTs) are partially man-made fats generally made by processing coconut and palm kernel oils in the laboratory. "Medium" refers to the chain length of the fatty acids. Oils can contain short chain, medium chain, or long chain fatty acids and most oils are a combination of all three types. Medium chain fatty acids contain between 6 and 12 carbon chains. They are: C6 caproic acid, C8 caprylic acid, C10 capric acid, and C12 lauric acid. MCTs passively diffuse from the GI tract to the portal system without requirement for modification like long-chain fatty acids or very-long-chain fatty acids. MCTs do not require bile salts for digestion. Patients who have malnutrition, malabsorption or particular fatty-acid metabolism disorders are treated with MCTs, as MCTs do not require energy for absorption, use, or storage. They have potentially beneficial attributes in protein metabolism, but may be contraindicated in some situations due to a reported tendency to induce ketogenesis and metabolic acidosis. Medium chain triglycerides (MCTs) are partially man-made fats generally made by processing coconut and palm kernel oils in the laboratory. "Medium" refers to the chain length of the fatty acids. Oils can contain short chain, medium chain, or long chain fatty acids and most oils are a combination of all three types. Medium chain fatty acids contain between 6 and 12 carbon chains. They are: C6 caproic acid, C8 caprylic acid, C10 capric acid, and C12 lauric acid. MCTs passively diffuse from the GI tract to the portal system without requirement for modification like long-chain fatty acids or very-long-chain fatty acids. MCTs do not require bile salts for digestion. Patients who have malnutrition, malabsorption or particular fatty-acid metabolism disorders are treated with MCTs, as MCTs do not require energy for absorption, use, or storage. They have potentially beneficial attributes in protein metabolism, but may be contraindicated in some situations due to a reported tendency to induce ketogenesis and metabolic acidosis. Foods that are good sources of valine include cottage cheese, fish, poultry, peanuts, sesame seeds, and lentils. In sickle cell disease, the replacement of an A by T in the beta chain gene of hemoglobin changes the codon GAG (for glutamic acid) to GTG (which encodes valine). Thus valine, a hydrophic amnio acid, replaces glutamic acid, a hydrophilic amino acid, causing hemoglobin to not fold correctly. It is also a precursor in the penicillin biosynthetic pathway.

Glutamine, (L-glutamine) is an amino acide traditionally considered nonessential, however it may be considered "conditionally essential" after critical illness, stress, or injury. (Glutamine is different than glutamate (glutamic acid), monosodium glutamate, and gluten.) It is the most abundant free amino acid in human muscle and plasma, and is important as a constituent of proteins and as a means of nitrogen transport between tissues. Muscle is the major tissue involved in glutamine synthesis. Glutamine is utilized at high rates by leukocytes (particularly lymphocytes) to provide energy and optimal conditions for nucleotide biosynthesis, and therefore cell proliferation metabolic requirements. In times of stress (illness or prolonged exercise for example) there is an associated decrease in the intramuscular and plasma concentrations of glutamine, and it has been hypothesized that this decrease in glutamine availability could impair immune function. Glutamine is utilized at high rates by leukocytes (particularly lymphocytes) to provide energy and optimal conditions for nucleotide biosynthesis, and therefore cell proliferation metabolic requirements. In times of stress (illness or prolonged exercise for example) there is an associated decrease in the intramuscular and plasma concentrations of glutamine, and it has been hypothesized that this decrease in glutamine availability could impair immune function. Dietary sources of glutamine include plant and animal proteins such as beef, pork, poultry, milk, yogurt, ricotta cheese, cottage cheese, raw spinach, raw parsley, and cabbage. PMID: 16215069
