Term Description
Detects E6/E7 viral messenger RNA (mRNA) of human papillomavirus (HPV) types 18 and 45 in cervical specimens from women with positive HPV assay results. E6/E7 oncogenes must be expressed for malignant transformation. For women 21-years and older with atypical squamous cells of undetermined significance (ASC-US) cervical cytology results, and HPV positive assay results, this assay can be used to assess the presence or absence of high-risk HPV genotypes 18, and/or 45. For women 30-years and older, the HPV 18, and/or 45 genotype assay can be used to test samples from women with HPV assay positive results, along with cervical cytology results. This assay has not been evaluated for HPV vaccinated patients or in cases of suspected abuse. It does not differentiate HPV 18 and HPV 45. This term was created for, but not limited in use to, the Aptima HPV 16 18/45 genotype assay which uses target capture, target amplification by transcription-mediated amplification and detection of the amplification products by hybridization protection. HPV type 16 and 18 are the most common carcinogenic HPV types. HPV type 16 is detected in about 50% of invasive cervical cancers worldwide and is associated with a higher risk of a severe cervical intraepithelial neoplasia (CIN 3). HPV type 16 is one of the greatest risks for HPV persistence along with increasing age, smoking, immunosuppression, inflammation, hormones, and genetic factors. HPV type 18 is detected in 10% to 20% of invasive cervical cancers and more commonly found in adenocarcinomas compared to squamous-cell cervical carcinomas. PMID: 19061814

Human papilloma virus (HPV) is a double-stranded DNA virus that includes 120 types that infect cutaneous and mucosal epithelial cells. The HPV that infect the cervix are categorized as low or high risk depending upon their carcinogenicity. High risk HPV types can lead to low-grade cervical cell abnormalities, pre-cancerous cellular abnormalities and anogenital cancer. Type 16 is a high risk HPV and is implicated in roughly 50% of cervical cancers around the world. E6/E7 genes in cervical tumors interfere with the control of cell proliferation and cell division and can be used for targeted anti-cancer therapy. PMID: 9448307
