Term Description
This term is used to report the presence or absence of glycopeptide resistant Staphylococcus aureus based on inhibitory concentrations of vancomycin and teicoplanin. This term was created for, but is not limited in use to, bioMerieux's ETEST GRD assay, which is intended to identify but not differentiate between Staph aureus strains with intermediate or heterogeneous resistance, called GISA and hGISA, respectively. To report which of GISA or hGISA is present, use [LOINC: 85042-0].

A gradient strip is a reagent strip that contains a predefined concentration gradient of a specific antibiotic or combination of antibiotics that is used for antimicrobial susceptibility testing (AST) and antimicrobial resistance detection (ARD).
For AST, the result is reported in terms of the minimum inhibitory concentration (MIC) of a specific antimicrobial that prevents the visible growth of bacteria. In LOINC, the "Gradient strip" method is used for AST gradient strips that result in an MIC result.
For ARD, the result is reported as the presence or absence of a specific enzyme or other mechanism that confers resistance, which is determined based on comparing the inhibitory concentration (IC) of a specific antimicrobial with the IC of that antimicrobial plus a resistance inhibitor. In LOINC, the "Gradient strip ARD" method is used for ARD gradient strips, which are not intended for determining individual MIC values.

Term Description
This term is used to report the presence of Staph aureus strains with intermediate or heterogeneous glycopeptide resistance, called GISA and hGISA respectively. One method for differentiating GISA from hGISA is based on comparison of inhibitory concentration results for vancomycin plus teicoplanin [LOINC: 85045-3] with the result for vancomycin alone [LOINC: 7059-9]. This term was created for, but is not limited in use to, bioMerieux's ETEST GRD assay, which, when used together with a vancomycin susceptibility gradient strip, can differentiate between GISA and hGISA.
