Neutrophils are white blood cells that are critical to the healthy function of the immune system. They are drawn to sites of infection and injury to phagocytose and digest microbes. PMID: 22224774 Neutrophils rely on their ability to generate an oxidative burst to effectively kill microorganisms. The oxidative burst is a series of reactions involving NADPH oxidase that is triggered by receptors when they encounter bacterial peptides such as N-formyl-Met-Leu-Phe (fMLP) and phorbol myristate acetate (PMA) and which results in the production of superoxide anions and reactive oxygen species (ROS). PMID: 22262438 PMID: 10733101 The inability of neutrophils to create an oxidative burst can lead to a decreased ability to fight infection. Chronic granulomatous disease (CGD) is an x-linked or autosomal-recessive disorder where NADPH oxidase is improperly assembled which leads to an inadequate oxidative burst response and increased susceptibility to bacterial and fungal infections. PMID: 22262438 One method of assessing neutrophil function uses flow cytometry to measure the fluorescence of neutrophils that have been stimulated in the presence of the nonfluorescent dye dihydrorhodamine 123 (DHR 123), which, when oxidized, produces the fluorescent compound rhodamine 123. Both the percent of neutrophils having a burst response and the efficacy of that response as reflected by the mean fluorescence intensity (MFI) is measured. Depending on the underlying genetic cause and severity of CGD, some patients have a population of neutrophils that can generate a burst response but with lower MFI compared to healthy controls, while others only have neutrophils that are not capable of generating a response at all.
