Optical coherence tomography (OCT) is a non-invasive imaging technique that provides an in vivo, cross-sectional view of the retina. There are 6 retinal layers viewable with OCT: the nerve fiber layer (NFL), the inner plexiform layer and ganglion cell layer (IPL+GCL), the inner nuclear layer (INL), the outer plexiform layer (OPL), the outer nuclear layer and photoreceptor inner segments (ONL+PIS), and photoreceptor outer segments (POS). OCT uses low-coherence interferometry to provide very accurate cross-sectional images. Two forms of OCT are time domain and spectral domain. Spectral domain has replaced the former because of its speed (20,000+ scans per second to avoid motion artifacts) and high resolution, and it can both visualize and measure the thickness of the retinal pigment epithelium and the nerve fiber layer adjacent to the optic disc. The RNFL is formed from expansion of the fibers of the optic nerve and extends to the interior surface of the retina. The RNFL is divided into 4 quadrants when measured in each eye - temporal, superior, nasal, and inferior. Each 90 degree quadrant is further divided into three 30 degree sectors ("clock hours"). RNFL measurements include the thickness of each quadrant and the thickness of each sector. The mean thickness of the entire RNFL for each eye is calculated based on the individual measurements. In patients with glaucoma, RNFL changes in the inferior quadrant are most closely associated with glaucoma status.
