Lipoprotein associated phospholipase A2 (Lp-PLA2) is an enzyme currently being evaluated to assist in risk factor assessment of cardiovascular disease. This biomarker was previously known as platelet-activating factor acetylhydrolase (PAF-AH) and has two biological functions. One function is that Lp-PLA2 inactivates an important proinflammatory mediator known as platelet-activating factor (PAF). The other function of Lp-PLA2 is that is oxidatively hydrolyzes modified polyunsaturated fatty acids resulting in the chemical compounds lysophosphatidylcholine (LysoPC) and oxidized nonesterified fatty acids (OxNEFA). OxNEFA has powerful monocyte chemotactic activity. LysoPC increase inflammatory mediators such as cytokines, adhesion molecules and the chemotactic mediator MCP-1. LpPLA2 is positively associated with coronary disease but does not improve cardiovascular risk assessment over the traditional biomarkers. LpPLA2 has been more useful for assessing patients with a borderline risk than traditional biomarkers. LpPLA2 has been especially helpful when used to measure cardiac risk on patients with metabolic syndrome or diabetes. There is some speculation that LpPLA2 may eventually be used to monitor treatment efficacy. Future risk assessments for coronary disease will likely include multiple combinations of cardiac risk markers. PMID: 17892360 RefID 23065
