Parathyroid hormone (PTH) analysis in rinsed material obtained from fine-needle aspiration biopsy (FNAB) helps to discriminate thyroid tissues from enlarged parathyroid glands and also facilitates parathyroid localization prior to surgery. Various groups have reported on the utility of this technique with specificity of 91% to 100% and sensitivity of 91% to 100%. Measuring PTH in the rinse material proved very useful in cases of non-diagnostic cytology. Comparing the results of the PTH rinse material with serum PTH is highly recommended. An elevated PTH in the serum could falsely elevate PTH in the washings if the rinse is contaminated with blood. In these cases, only PTH values significantly higher than the serum should be considered as true positives.

Parathyrin intact (iPTH) levels are requested for the differential diagnosis of calcium-related abnormalities. The test is useful for diagnosis of primary hyperparathyroidism, secondary hyperparathyroidism, and differential diagnosis of hypercalcemia. Because there are many fragments of PTH, tests for PTH may measure one or more of the fragments. None of the assays for intact PTH measure PTH (35-84), which is actually the fragment of PTH present in highest amounts in blood. Many intact PTH assays measure either PTH (1-84) or PTH (7-84). In most people, PTH (7-84) is present in much lower amounts than PTH (1-84), so this is not a concern. But in kidney failure, PTH (7-84) levels increase compared to PTH (1-84), and sometimes over half of what is measured as PTH represents this N-terminal truncated fragment. Some intact PTH assays do not measure this fragment and will give lower PTH results when increased PTH (7-84) is present.
