This term was created for, but is not limited in use to, the submitter's test strip, a membrane strip pre-coated with a specific set of antigens in parallel lines. If antibodies to any of those antigens are present in the patient's serum, they will bind to the specific antigen. The bound patient antibodies are then detected using labelled anti-human antibodies. Various manufacturers classify this method as an immunoblot technique. LOINC uses the immunoblot (IB) method to mean that some electrophoretic/chromatographic separation occurs such that position (size) of the band is part of the assessment (as in Western blot). Centromere protein A, also known as Histone H3-like centromeric protein A (CENP-A), replaces conventional H3 in the nucleosome core of centromeric chromatin at the inner plate of the kinetochore. It is required for recruitment and assembly of kinetochore proteins, progression of mitosis and segregation of chromosomes. [UniProt: P49450] Anti-CENP-A antibodies are a marker of systemic sclerosis (SSc), a multisystem autoimmune connective tissue disorder that is characterized by vascular abnormalities, excessive collagen synthesis, and autoantibodies to nuclear and nucleolar proteins. Approximately 20-40% of patients with SSc are positive for anti-CENP-A or-B antibodies, and these patients are at higher risk of developing pulmonary arterial hypertension, but a lower risk of early interstitial lung disease, at least early in the disease course. Centromere protein A (CENP-A) is an essential centromere protein encoded by the CENPA gene. Overexpression of CENP-A causes chromosomal aberrations, and depletion of CENP-A promotes apoptosis and induces cell cycle arrest. Overexpression of CENP-A has been associated with high grade cancers and is a prognostic factor for patients with estrogen receptor (ER)-positive breast cancer. Antibodies to CENP-A are associated with scleroderma, systemic sclerosis, CREST syndrome, and other rheumatologic diseases.

Major centromere autoantigen B, also known as centromere protein B or CENP-B, is encoded by the CENPB gene located on chromosome 20p13. CENP-B interacts with chromosomal centromeric heterochromatin and binds to a specific subset of alphoid satellite DNA, called the CENP-B box. Anti-fibrillarin antibody (AFA), also known as anti-U3-RNP, is an antibody to a nucleolar ribonucleoprotein called fibrillarin, which marks the site of formation of the nucleolus in cells that are dividing. AFA is associated with systemic sclerosis (scleroderma) and is more prevalent in African American patients with scleroderma compared to other patient populations. In addition, patients with scleroderma who have AFA tend to be younger at disease onset and have a higher frequency of diffuse cutaneous involvement, pulmonary artery hypertension, musculoskeletal and cardiac involvement, and lower frequency of arthritis. Human upstream binding factor (hUBF) is a nucleolar protein that plays a key role in rRNA production. The protein regulates rDNA transcription and also promotes rRNA synthesis. DNA binding to hUBF has the potential to alter chromatin by inducing chromatin remodeling. POL3RA gene encodes the largest core component (known as RP155) of RNA polymerase III, which synthesizes small RNAs such as 5S rRNA and tRNAs. POL3RA protein is part of the active center of the polymerase. The POL3RA protein plays a key role in sensing and reducing infection by intracellular bacteria and DNA viruses. It also acts as a sensor involved in innate immune response. Term Description
This term was created for, but is not limited in use to, the submitter's test strip, a membrane strip pre-coated with a specific set of antigens in parallel lines. If antibodies to any of those antigens are present in the patient's serum, they will bind to the specific antigen. The bound patient antibodies are then detected using labelled anti-human antibodies. Various manufacturers classify this method as an immunoblot technique. LOINC uses the immunoblot (IB) method to mean that some electrophoretic/chromatographic separation occurs such that position (size) of the band is part of the assessment (as in Western blot). Because this assay does not involve a separation step but rather uses strips with pre-fixed antigens, the LOINC Method is Line blot.

Patient's with Th/To autoantibodies immunoprecipitate two small RNAs, known as 7-2 RNA (also RNAase MRP or Th) and 8-2 RNA (also RNAase P, H1 and To). 7-2 RNA is involved in processing of pre-rRNA and 8-2 RNA is involved in maturation of tRNA. Anti-Th/To is frequently detected in the sera of patients with scleroderma and their presence may be associated with a less favorable prognosis.

Th/To are small nuclear ribonucleoproteins located in the nucleoli of cells. Anti-Th/To antibodies, a type of antinuclear antibodies (ANA), are present in a small subset (2-5%) of patients with with limited cutaneous systemic sclerosis (lSSc). But they present a higher risk for severe organ involvement and therefore a worse prognosis. Severe pulmonary fibrosis followed by pulmonary hypertension have been reported.

The U1 snRNP complex is responsible for pre-mRNA processing. Autoantibodies to different components of the U1 snRNP complex are associated with autoimmune conditions including systemic lupus erythematosus (SLE) and mixed connective tissue disorders (MCTD), which includes systemic sclerosis. The pattern of autoantibodies present in an individual patient may help distinguish between different connective tissue disorders. For example, patients with anti-U1A and anti-U1C IgM antibodies are more likely to have SLE than MCTD. PMID: 24158973 In addition, there is some evidence that specific antibody patterns are associated with specific symptoms. For example, among patients with SLE, the combination of anti-U1-70kD, anti-U1A and anti-U1C correlates with Raynaud's phenomenon, while anti-U1-70kD and anti-U1A (without anti-U1C) correlate with leukopenia. Another study found an association between the number of specific positive antibodies within the anti-U1 snRNP (rather than the presence of particular markers) and certain clinical features. For example, patients with three or more positive autoantibodies were more likely to have features of systemic sclerosis and less likely to have renal disease compared to patients with fewer types of antibodies.
