Term Description

The 4Kscore aggressive cancer risk score is determined by an algorithm that uses four prostate-specific and kallikrein test results (total prostate specific antigen (PSA), free PSA, intact PSA, and human kallikrein) and other specific clinical information to predict a patient's risk for aggressive prostate cancer. The 4Kscore assay is used to help guide the decision of whether or not a biopsy should be performed based on a patient having an elevated prostate specific antigen (PSA) test or abnormal digital rectal exam. Currently, the decision to have a biopsy based on PSA levels has a low specificity and high false-positive rate for aggressive prostate cancer. Fifty percent of all prostate biopsies performed find no cancer, and 25% of all prostate biopsies performed find an indolent, non-life threatening cancer. Patients with a 4Kscore of greater than or equal to 7.5% would be referred for a biopsy. Patients with a 4Kscore less than 7.5% are considered low risk for aggressive prostate cancer and would not be referred for a biopsy. PMID: 28522924
