Term Description
The DNA amplification target is the 283-bp plasminogen-binding protein gene (oppA1), which is present at a frequency of one copy per organism in all four pathogenic species of the Borrelia burgdorferi sensu lato genogroup (Borrelia burgdorferi sensu stricto, Borrelia afzelii, Borrelia garinii, and Borrelia mayonii). Melting curve analysis is used to identify B. mayonii and differentiate it from the other three species.

Borrelia mayonii is the proposed name for a bacterial species found in 2013, causing Lyme disease in six people who live in the upper Midwestern United States. Worldwide, Lyme disease is caused by three main species of bacteria: Borrelia burgdorferi, B. afzelii, and B. garinii. Borrelia burgdorferi causes Lyme disease in the United States, while the other two species are the leading causes of Lyme disease in Europe and Asia. B. mayonii is a new species that is genetically distinct from these three species and is the only species besides B. burgdorferi shown to cause Lyme disease in North America. B. mayonii has been identified in blacklegged (or "deer") ticks collected in several counties in northwestern Wisconsin and Minnesota. It is highly likely, however, that infected ticks occur in other areas of both states. Limited information suggests that illness caused by B. mayonii is similar to that caused by B. burgdorferi, but with a few possible differences. Like B. burgdorferi, B. mayonii causes fever, headache, rash, and neck pain in the early stages of infection (days after exposure) and arthritis in later stages of infection (weeks after exposure). Unlike B. burgdorferi however, B. mayonii appears to be associated with nausea and vomiting, diffuse rashes, and a higher concentration of bacteria in the blood. Patients infected with B. mayonii have been successfully treated with a 2-4 week course of doxycycline. Amoxicillin, ceftriaxone, and cefuroxime have also been used.

Lyme disease is a multisystem, multistage infection caused by three species of tick-borne spirochetes in the Borrelia burgdorferi sensu lato genogroup. These spirochetes include B. burgdorferi sensu stricto (North America and Western Europe), B. afzelii (Central and Western Europe and Russia), and B. garinii (Europe, Russia, and northern Asia). Endemic areas for Lyme disease in the United States correspond with the distribution of two tick species, Ixodes dammini (Northeastern and Upper Midwestern US) and Ixodes pacificus (West Coast US). In Europe, Ixodes ricinus transmits the spirochete. Human Lyme borreliosis, the most prevalent arthropod-borne infection in temperate climate zones around the world, is an inflammatory disorder characterized by the skin lesion erythema migrans and the potential development of neurologic, cardiac, and joint abnormalities. B. garinii, the most neurotropic of these three genospecies, causes most neurologic Lyme disease in Europe, including cases of meningopolyneuritis and, rarely, encephalomyelitis. Although probably all three genospecies can cause all major manifestations, it appears that infection with different B. burgdorferi sensu lato genospecies results in distinct presentations of Lyme borreliosis. In Europe, B. afzelii is mostly associated with skin manifestations, such as erythema migrans, and acrodermatitis chronica atrophicans, whereas B. garinii is the main cause of Lyme neuroborreliosis. Term Description
The DNA amplification target is a gene encoding glycerophosphodiester phosphodiesterase (GlpQ), which is specific to the Borrelia species in the relapsing fever group. This gene is not found in the Borrelia species that cause Lyme disease. Melting curve analysis is then used to identify the organism as B. miyamotoi. A positive result is consistent with active or recent infection.

Borrelia miyamotoi is a spirochetal bacterium that is closely related to the Borrelia species that causes tick-borne relapsing fever (TBRF), and it is more distantly related to the Borrelia species that cause Lyme disease. First identified in 1995 in ticks from Japan, B. miyamotoi has since been detected in two species of North American ticks, the black-legged or "deer" tick (Ixodes scapularis) and the western black-legged tick (Ixodes pacificus). These ticks are already known to transmit several diseases, including Lyme disease, anaplasmosis, and babesiosis. Patients with B. miyamotoi infection are most likely to have fever, chills, and headache. Other common symptoms included body and joint pain and fatigue. Unlike Lyme disease, rash is uncommon, presenting in only 4 out of 51 patients. Patients infected with B. miyamotoi have been treated with a 2-4 week course of doxycycline, amoxicillin, or ceftriaxone. Fewer than 60 well-documented cases of human infection have been reported in the United States as of July 2015. The route of transmission of B. miyamotoi to humans has not been firmly established, but likely involves the bite of infected black-legged ticks. The glpQ gene encodes a periplasmic enzyme, glycerophosphodiester phosphodiesterase of 333 amino acids produced initially with a 25 residue long signal sequence. GlpQ is not found in Borrelia species that cause Lyme disease.
