Part Descriptions
LP411435-3 Beta hydroxythiofentanyl
The compounds known as novel synthetic opioids (NSOs) have been emerging as drugs of abuse, given that many share pharmacological properties with common opioid agonists, yet they are difficult to detect and control. Since the early 2010s clandestine manufacturing of both novel fentanyl derivatives as well as other synthetic opioids has increased, with the list growing to upwards of more than 30 unique compounds. The list includes fentanyl analogs such as acetylfentanyl, butyrfentanyl, 4-methoxybutyrylfentanyl, 4-methylphenethylacetylfentanyl, furanylfentanyl, carfentanil (and its major metabolite norcarfentanil), ocfentanil, acrylfentanyl, 3-methylfentanyl and 4-fluorobutyrylfentanyl. Also, in the same category are non-fentanyl derived chemicals having opioid agonist activity such as AH-7921 and U47700(benzoamides) and MT-45 (a piperazine). The pharmacologic receptor profile of some predict that they would be highly addictive and in some cases much more potent (for example, 3-Methylfentanyl is about 1000 times and carfentanil may be 10,000 times more potent than morphine). The fact that they are often cut into heroin preparations or sold as counterfeit pain killers with users unaware of their content makes them particularly prone to overdose. The DEA has reported that the fentanyl crisis in the United States in the late 2010s has been largely fueled by NSOs PMID: 30634521.

93471-1 Acetyl norfentanyl [Mass/volume] in Urine by Confirmatory method
Part Descriptions
LP248692-8 Acetyl norfentanyl
Acetyl norfentanyl is the primary metabolite of acetyl fentanyl via hepatic cytochrome P450. One method used to detect acetyl norfentanyl is solid phase extraction followed by liquid chromatography-tandem mass spectrophotometry.[http://www.who.int/medicines/access/controlled-substances/5.2_Acetylfentanyl_CRev.pdf] Acetyl fentanyl is a highly potent designer drug and a chemical relative of fentanyl. Acetyl fentanyl is five times stronger than heroin and use of it can lead to life-threatening symptoms including decreased consciousness, respiratory depression, cerebral hemorrhage and miosis. 93463-8 Ocfentanil [Mass/volume] in Urine by Confirmatory method
Part Descriptions
LP411414-8 Ocfentanil

LP411334-8 AH-7921
AH-7921 (3,4-dichloro-N-{[1-(dimethylamino)-cyclohexyl] methyl}benzamide) is a novel synthetic opioid agonist, first developed in the 1970s, but never used clinically due to its strong addictive properties. It shares a benzamide chemical structure with several other synthetic opioids such as U-47700, and is an emerging drug of abuse PMID: 29973882.

93459-6 U-47700 [Mass/volume] in Urine by Confirmatory method
Part Descriptions
LP411333-0 U-47700
U-47700 is an novel synthetic opioid that is a selective μ receptor-agonist with a potency 7.5 times greater than morphine. It is structural isomer of AH-7921, another synthetic opioid with similar characteristics. U-47700 was originally studied in the 1970s by the Upjohn Company as a potential analgesic but was never brought to market and has become an emerging drug of abuse. It is known on the street as "pink" due to its pinkish hue when in pure form, although it is found cut into preparations of heroin (a combination known as "gray death") or fentanyl. Common effects are analgesia, sedation, euphoria, itching, and miosis. Because of its receptor affinity (μ-agonist), it has a high risk of addiction and respiratory depression PMID: 30849683.

MT-45 is a new generation synthetic opioid with a piperidine structure (unrelated to most other opioids) that has appeared on recreational drug market. It is 3.5 times more potent than morphine and is found in mixtures such as "Wow" (with synthetic cathinones). MT-45 has been shown to cause physical dependence in mice, and adverse effects reported in humans include hair loss, dermatitis, elevated liver enzymes, cataracts, hearing loss and death PMID: 30634521.

W-18 (4-chloro-N-[1-[2-(4-nitrophenyl)ethyl]-2-piperidinylidene]-benzenesulfonamide) and W-15 (4-chloro-N-[1-(2-phenylethyl)-2-piperidinylidene]-benzenesulfonamide) are emerging drugs of abuse. Although they have not been shown to have specific opioid receptor activity PMID: 29202454, they are structurally related to the opioid agonist fentanyl. W-18 and W-15 are 2 of a series of substances (named W-1 through W-32) that were synthesized in the 1980s as possible analgesics. W-19 is another similar compound that classified as an emerging drug of abuse. At the time of this writing the literature is unclear about the potency of these compounds and their dealkylation metabolites, nor-W-18, nor-W-15 and nor-W-19.
