Rh is the most complex of the blood groups systems, encompassing over 45 distinct antigens. The most clinical important antigen, D or Rho, was first described in 1940 and is present in over 85% of the population. Those individuals who lack the D antigen are considered to be Rh negative.
The Rh antigens are encoded by two highly homologous and closely linked genes on the short arm of chromosome 1. The RHD gene codes for the D antigen, or most of its components; RhCE gene codes the Cc and Ee antigens or their variants. The majority of the antigens within this system represent products of gene cross-over, point mutations or deletions within one or both genes.
The Rh antigens appear to be expressed only on red blood cell and they appear early during red cell development. Antibodies against the Rh antigens have caused severe and fatal transfusion reactions and hemolytic disease of the newborn. Rh antigens have been implicated in many diseases such as auto-immune hemolytic anemia, auto-Rh antibodies. Collect: One 4 mL green (sodium or lithium heparin) or serum separator tube. Also acceptable: lavender or pink (EDTA).

Kell is highly polymorphic. It has two major codominant alleles, k and K, which result from a SNP (698C→T), and the corresponding k and K antigens differ by a single amino acid change (T193M). Mostly IgG antibodies, so IgM is uncommon. Anti-K does not bind complement. If hemolysis does occur, it is extravascular in nature. Anti-K can cause a severe hemolytic transfusion reaction and severe fetal anemia. In the RBC membrane, the Kell glycoprotein is covalently linked to the XK protein, a multipass membrane protein thought to have a role in transport. In the absence of XK, a condition called McLeod syndrome, Kell antigens are only weakly expressed and the RBCs are abnormal with spiky projections (acanthocytosis). Systemic findings include muscular dystrophy, cardiomyopathy, psychiatric disturbances, and neurological defects, such as loss of reflexes and movement disorders. D antigen is part of the Rh blood group system. Most people have red blood cells that either express the D antigen (D+) or don't (D-), but there are some people that have a phenotype that does not strictly fall into either the D+ or D- category. There are many types of D variants. "Weak D" is a D variant in which all epitopes of the D antigen are expressed, but in smaller quantity than usual. Patients with the weak D variant do not typically have anti-D antibodies. "DEL" is a variant in which D expression is low enough that it cannot be identified using typical serology. "Partial D" is a variant in which only some epitopes of the D antigen are expressed. Patients with the partial D variant may have anti-D antibodies, specifically to those epitopes that are not expressed. Testing for D variants is especially important in patients who need chronic transfusions and pregnant women. PMID: 23432139
