Glial fibrillary acidic protein (GFAP) is an intermediate filament (IF) protein highly expressed in the central nervous system (CNS), almost exclusively in astrocytes, as well as in the peripheral nervous system (PNS). GFAP is used as a biomarker for traumatic brain injuries (TBIs) and stroke. In the presence of astrogliosis, GFAP gene activation and protein production are found to be increased. Additionally, studies have found that an increased amount of GFAP is found in serum following TBIs and stroke. PMID: 25975510

Ubiquitin carboxyl-terminal hydrolase-L1 (UCH-L1) is a protein that is involved in the metabolism of ubiquitin within neurons PMID: 10848595. Increases in blood UCH-L1 have been detected in the serum of mild and moderate TBI patients within an hour of injury PMID: 22673263. Levels measured within 4 hours of injury were significantly higher in those with TBI lesions on CT than those with a normal intracranial appearance on CT. Blood levels of UCH-L1 have been demonstrated to be able to discriminate mild TBI patients from patients without head injuries and, similar to GFAP, UCH-L1 levels were much higher in patients who required neurosurgical intervention PMID: 22673263.

Term Description
Overall combined interpretation (e.g. elevated, not elevated) of GFAP and UCH-L1 proteins in plasma specimens. If both are below the cutoff value, the overall result is not elevated. If one or both is equal to or above the cutoff, the overall result is considered elevated. Results in conjunction with other clinical information are used to determine the need for additional follow-up testing, including a head CT.
