LP6336-4 Immunofixation
Plasma or concentrated urine is deposited on a gel. With an electrical current the proteins are sorted in function of their size, after which antigens for each targeted type of immunoglobulin are deposited on the gel. Thus more or less narrow bands appear on the gel, marking the location of the various immunoglobulines.
Immunofixation tends to replace protein electrophoresis because:
• it is faster (results within three hours);
• it is somewhat more sensitive. Immunofixation may reveal an immunoglobulin missed out by protein electrophoresis, especially at low concentrations (less than 1 gram/litre);
• it can be partially automated and can be used in more laboratories;
• it is more easily read and interpreted.
Immunofixation is however only sensitive to immunoglobulins and is more expensive than protein electrophoresis.

A monoclonal band (M-spike, monoclonal protein isotype) is often found in patients with monoclonal gammopathies. Following protein electrophoresis, immunofixation is used to identify the immunoglobulin heavy chain and/or light chain. Monoclonal peaks greater than 3 g/dL and IgG or IgA are seen in patients with Multiple Myeloma (MM). A monoclonal peak greater than 3 g/dL and IgM is seen in patients with macroglobulinemia. An alternate methodology for identifying the monoclonal protein isotype is MALDI-TOF, in which known information regarding the unique mass of each isotype is used to determine and quantify the isotype present in the patient sample based on the mass spectra generated from the sample.
