Gastrin is a hormone that stimulates the release of gastric acid from the parietal cells in the stomach. It is secreted in response to stomach distension, vagal stimulation, the presence of partially digested proteins, and/or hypercalcemia. Gastrin is produced at high levels in Zollinger-Ellison syndrome, often by a gastrin-producing tumor (gastrinoma). Gastrinomas are usually found in the pancreas, but may be in the duodenum. Gastrinoma are malignant in 62% of cases, with 44% having metastases. Patients with Zollinger-Ellison syndrome have gastric and duodenal ulceration due to excessive, unregulated gastric acid secretion. 15-26% of Zollinger-Ellison patients have evidence of Werner syndrome-multiple endocrine neoplasia type 1.
